摘要
目的从长期照护服务供给与需求视角,探究广西医养结合养老机构入住失能老人的长期照护现状。方法采用典型调查法选取5家医养结合养老机构,通过问卷调查和访谈,了解失能老人养老需求和机构长期照护服务供给情况。结果超过半数失能老人患有1种以上疾病,且以慢性病为主,中风是造成老人失能的主要原因之一。失能老人对基本生活照料和院内专业的医疗护理需求较高。虽然机构探索出整合照料和联合运行等医养结合模式,但存在照护服务质量不高,医保报销难等问题。结论医养结合养老机构有望成为失能老人的最佳选择,但失能老人面临长期照护服务需求增长与供给不足、专业化高质量的长期照护服务需求与机构服务能力不足、质量不高的双重困境。建议尽快推行长期护理保险,明确医养结合养老机构功能定位,探索多元化医养结合养老方式。
Objective To explore the status quo of long-term care for the disabled elderly of medical caring and nursing combined institution for the aged in Guangxi based on the perspective of long-term care service supply and demand. Methods Typical investigation method was adopted to select five medical caring and nursing combined institution for the aged in Guangxi Zhuang Autonomous Region for a questionnaire survey and interview to explore the needs of the disabled elderly and the supply of long-term care services. Results More than half of the disabled elderly suffered from more than one kind of disease, which were mainly chronic diseases. Stroke was one of the main causes of the elderly’s disability. The disabled elderly had a high demand for basic life care and professional medical care in the institution. Although the institutions had explored the integrated care and joint operation model of medical and nursing care, there were problems such as low quality of care services and difficulty in reimbursing medical insurance. Conclusions The medical caring and nursing combined institutions for the aged are expected to be the best choice for the disabled elderly, but the reality is the imbalance between demand growth and insufficient supply, the dilemma of specialized, high-quality long-term care service needs and insufficient institutional service capacity and low quality. It is recommended to implement long-term care insurance as soon as possible, clarify the functional positioning of the medical caring and nursing combined institution for the aged, and explore various ways of combining medical care and elderly care.
