摘要
目的探讨基于锚定法的最小临床重要性差值MCID的合理计算方法,并制定胃癌量表(QLICP-GA)的MCID。方法分别以EORTC QLQ-C30的Q29和Q30为主观锚,计算患者出院时和入院时各领域得分差值,以锚定法"相差一个等级"、"至少相差一个等级"及"改善"三种标准、基于锚的MIC分布法、多元线性回归模型制定MCID。结果以Q29和Q30为主观锚具有较强的一致性,以"至少相差一个等级"为标准得到的MCID值介于另外两个标准之间。以Q29和Q30分别为锚时QLICP-GA生理功能、心理功能、社会功能、共性症状及副作用、特异模块、共性模块及量表总分MCID分别为7.14、6.25、4.17、14.29、14.29、7.81、8.97和7.14、7.29、4.17、14.29、16.84、7.81、8.33。结论每种方法各具优缺点,可用Q29或Q30作为效标,以"至少相差一个等级"作为标准制定MCID值。
Objective To explore the reasonable calculation methods and standards for minimal clinically important differences(MCID)by anchor-based methods,and to develop the MCID of the gastric cancer scale(QLICP-GA).Methods Taking Q29 and Q30 of EORTC QLQ-C30 as the subjective anchors,respectively,the difference of scores in each domain on the day at discharge and at admission was calculated.MCID was formulated by anchor-based method according to three standards including“one grade difference”,“at least one grade difference”and“improvement”,and by anchor-based MIC distribution method and multiple linear regression model.Results The MCID using Q29 and Q30 as the subjective anchors had strong consistency,and the MCID values obtained by taking“at least one grade difference”as the standard were between the other two standards.When Q29 and Q30 were used as subjective anchors,the MCID for physical function,psychological function,social function,common symptoms and side effects,the specific module,common module and total score were 7.14,6.25,4.17,14.29,14.29,7.81,8.97 and 7.14,7.29,4.17,14.29,16.84,7.81,8.33,respectively.Conclusion Each method has its ad-vantages and disadvantages.It is suggested to use Q29 or Q30 as anchor,with“at least one grade difference”being as definition standard to calculate MCID value.
