摘要
通过中国知网(CNKI)、维普中文科技期刊数据库(VIP)和万方数据库检索小柴胡汤或小柴胡汤加减治疗胆汁反流性胃炎、慢性浅表性胃炎、慢性胃炎的随机对照实验(RCTs),采用RevMan 5.3软件对纳入文献结果进行Meta分析,研究小柴胡汤治疗胃炎的临床疗效,并采用网络药理学分析小柴胡汤治疗胃炎的作用机制.结果显示,共纳入58篇RCTs文献,包括2538例胆汁反流性胃炎患者、1570例慢性浅表性胃炎患者和1148例慢性胃炎患者.Meta分析结果显示,小柴胡汤(包括小柴胡汤加减)治疗胃炎的显效率[OR=2.74,95%CI(2.41,3.12),Z=15.53,P<0.00001]和总有效率[OR=4.44,95%CI(3.73,5.29),P<0.00001]均显著优于对照组,治疗效果在3类胃炎亚型中无明显偏倚性(P>0.05).网络药理学分析结果显示,小柴胡汤中20个核心成分可靶向胃炎相关的52个靶点,涉及33个GO功效,32个信号通路.本研究结果表明,小柴胡汤治疗胃炎疗效确切,其机制涉及多种组分多个靶点,充分体现了中药复方调节生理稳态的综合优势.
This study is to systematically analyze the clinical efficacy and therapeutic mechanism of Xiaochaihu decoction(XCHD)in treating gastritis.Randomized controlled trials(RCTs)of XCHD or modified XCHD in the treatment of gastritis were retrieved through CNKI,VIP and wanfang databases,and the results of included literatures were Meta-analyzed with RevMan 5.3software.The therapeutic mechanism of XCHD in treating gastritis was analyzed by network pharmacology.58RCTs literatures including 2538cases of bile reflux gastritis,1570 cases of chronic superficial gastritis and 1148cases of chronic gastritis were used.The results of Meta-analysis showed that XCHD(including modified XCHD)was significantly more effective in treating three types of gastritis than the control group,with a statistically significant difference in excellence rate[OR=2.74,95%CI(2.41,3.12),Z=15.53,P<0.00001]and total effective rate[OR=4.44,95%CI(3.73,5.29),P<0.00001],but had no significant bias among three types of gastritis(P>0.05).Network pharmacological analysis showed that 20 core components of XCHD could target 52target proteins related to gastritis,involving 33GO effects and 32signal pathways.This study showed that XCHD is effective in the treatment of gastritis,and its mechanism involves multiple components and targets,which fully reflects the comprehensive advantages of TCM compounds in regulating physiological homeostasis.
