摘要
目的分析舒筋逐瘀外洗方在前交叉韧带(ACL)重建术后临床应用效果。方法选择ACL断裂90例,均行关节镜下ACL重建术,据术后治疗方法不同,分为观察组和对照组,每组各45例。观察组术后予康复训练联合舒筋逐瘀外洗方治疗,对照组术后予康复训练。两组均采用视觉模拟评分法(VAS)评估术后2、6、12周患侧膝关节疼痛情况,利用膝关节Lysholm评分表对术后2、6、12周患侧膝关节功能进行评估,并观察术后2、6、12周患侧膝关节肿胀程度、患侧膝关节活动度,比较恢复工作时间,记录并发症发生情况。结果与对照组术后6周比较,观察组术后6周患侧膝关节VAS评分及患侧膝关节肿胀值降低,患侧膝关节活动度增加,患侧膝关节Lysholm评分表评分升高,差异有统计学意义(P<0.01)。观察组恢复工作时间短于对照组,差异具有统计学意义(P<0.01)。两组治疗期间均无伤口感染、呼吸系统感染、愈合延迟等并发症发生,且两组伤口均为甲级愈合。术后12周两组Lachman试验及前抽屉试验均阴性,膝关节功能恢复正常。结论ACL重建术后进行舒筋逐瘀外洗方熏洗干预可加速患者康复进程,促进膝关节功能恢复,且安全性较高。
Objective To analyze the clinical application value of Shujin Zhuyu Lotion after anterior cruciate ligament(ACL)reconstruction.Methods A total of 90 patients with ACL rupture were selected,and underwent arthroscopic ACL reconstruction.They were divided into observation group(n=45)and control group(n=45)according to different postoperative treatment methods.The observation group received rehabilitation training combined with Shujin Zhuyu Lotion treatment after operation,and the control group received rehabilitation training after operation.The visual analog scale(VAS)was used to evaluate the pain of the affected knee at 2,6,and 12 weeks after operation in both groups,and the Lysholm knee scoring scale was used to evaluate the function of the affected knee at 2,6,and 12 weeks after operation.The degree of swelling of the affected knee joint and the range of motion(ROM)of the affected knee joint at 2,6,and 12 weeks after operation were observed,and the time to resume work was compared.In addition,the occurrence of complication was recorded.Results Compared with the control group at 6 weeks after operation,the VAS score and swelling degree of affected knee joint were decreased in observation group at 6 weeks postoperatively,while the ROM and the Lysholm scoring scale of the affected knee joint were increased(P<0.01).The time to resume work was shorter in observation group than in control group(P<0.01).During the treatment period,there were no complications such as wound infection,respiratory system infection or delayed healing,and all wounds reached class A healing.There were positive results in Lachman test and anterior drawer test in both groups at 12 weeks after operation,and knee function returned to normal.Conclusion The intervention with Shujin Zhuyu Lotion for the fumigation and washing in patients after ACL reconstruction can effectively accelerate the rehabilitation process of patients,promote knee function recovery,and has the advantages of better safety.
