慢性阻塞性肺疾病急性发作期患者BUN、Hcy、BNP水平变化及其与血气分析指标的相关性 被引量:16

Changes of BUN,Hcy and BNP Levels in Patients with Chronic Obstructive Pulmonary Disease during Acute Attack and Their Correlation with Blood Gas Indicators

摘要 目的探讨慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)急性发作期患者血尿素(blood urea nitrogen,BUN)、同型半胱氨酸(homocysteine,Hcy)、脑利钠肽(rain natriuretic peptide,BNP)水平变化及其与血气分析指标的相关性。方法选取2018年1月—2019年12月收治的COPD急性发作期151例作为观察组,选取同期健康体检者62例作为对照组,比较两组血清相关指标(BUN、Hcy、BNP)、肺功能相关指标[第1秒用力呼气容积(expiratory volume in first second,FEV1)、用力肺活量(forced vital capacity,FVC)和FEV1/FVC]水平,分析COPD急性发作期患者BUN、Hcy、BNP与动脉血氧分压(arterial partial pressure of oxygen,PaO 2)、动脉血二氧化碳分压(arterial partial pressure of carbon dioxide,PaCO 2)的相关性。结果与对照组比较,观察组BUN、Hcy、BNP水平升高,FEV1、FVC及FEV1/FVC水平降低,差异有统计学意义(P<0.01)。Pearson相关性分析显示,COPD急性发作期患者PaO 2水平与BUN、Hcy、BNP水平呈负相关(P<0.01),PaCO 2水平与BUN、Hcy、BNP水平呈正相关(P<0.01),BUN水平与Hcy、BNP水平呈正相关(P<0.05或P<0.01),Hcy水平与BNP水平呈正相关(P<0.01)。结论COPD急性发作期患者BUN、Hcy、BNP水平显著升高,可能参与COPD急性发作期的发生发展过程。 Objective To investigate the changes in the levels of blood urea nitrogen(BUN),homocysteine(Hcy)and brain rain naturetic peptide(BNP),and their relationship with blood gas indicators in patients with chronic obstructive pulmonary disease(COPD).Methods A total of 151 patients with COPD during acute attack who were treated between January 2018 and December 2019 were enrolled as observation group,and 62 healthy physical examinees during the same period served as control group.Serum-related indicators(BUN,Hcy,BNP),pulmonary function-related indicators forced expiratory volume in first second(FEV1)and forced vital capacity(FVC),FEV1/FVC were compared.The correlation of BUN,Hcy,and BNP with arterial partial pressure of oxygen(PaO 2)and arterial partial pressure of carbon dioxide(PaCO 2)was analyzed in patients with acute COPD(AECOPD).Results Compared with the control group,BUN,Hcy and BNP levels in the observation group were increased,while FEV1,FVC and FEV1/FVC levels were decreased(P<0.01).Pearson correlation analysis showed that PaO 2 levels in patients with acute exacerbation of COPD were negatively correlated with BUN,Hcy and BNP levels(P<0.01),the level of PaCO 2 was positively correlated with BUN,Hcy and BNP(P<0.01),BUN level was positively correlated with Hcy and BNP levels(P<0.05 or P<0.01),and Hcy level was positively correlated with BNP level(P<0.01).Conclusion The levels of BUN,Hcy,and BNP in patients with COPD during acute attack are significantly increased,and may be involved in the occurrence and development of acute exacerbation COPD disease during acute attack.
