摘要
[目的]观察半夏白术天麻汤联合西药治疗颈动脉粥样硬化风痰阻络证的临床疗效,分析其临床应用价值。[方法]选取2015年6月—2018年6月在北京中医药大学东直门医院接受治疗的80例颈动脉粥样硬化风痰阻络证患者,通过随机数表法将患者随机分为观察组与对照组,各40例。对照组采用阿托伐他汀钙片进行治疗,观察组在此基础上加用半夏白术天麻汤,两组患者均连续治疗6个月,比较两组患者的疗效情况,颈动脉内-中膜厚度(IMT)、颈动脉斑块Crouse积分、血清各项指标水平,并分析治疗的安全性。[结果]观察组患者在治疗3个月及6个月时的临床疗效均明显优于对照组(P<0.05)。治疗6个月后,两组患者IMT与Crouse积分均显著下降,观察组上述两项指标均明显低于对照组(P<0.05)。治疗6个月后,观察组三酰甘油(TG)、血清总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)水平均明显低于对照组(P<0.05)。治疗6个月后对两组患者进行常规的血、尿、肝肾功能、心电图等检查,均无出现明显变化,两组患者在治疗过程中均未出现严重不良反应。[结论]半夏白术天麻汤联合西药治疗颈动脉粥样硬化风痰阻络证的疗效显著,且安全性较高,值得进行临床推广。
[Objective]To observe the clinical efficacy of Banxia Baizhu Tianma Decoction combined with Western medicine in the treatment of carotid atherosclerosis with wind-phlegm blocked collaterals syndrome,and analyze its clinical application value.[Methods]The 80 cases of carotid atherosclerosis with wind-phlegm blocked collaterals syndrome who were treated in our hospital from June 2015 to June 2018 were selected and randomly divided into observation group and control group,with 40 cases in each group.The control group treated with atorvastatin calcium tablet,the observation group was additionally treated with Banxia Baizhu Tianma Decoction.Two groups of patients were treated continuously for 6 months.The curative effect,carotid intima-media thickness(IMT),carotid plaque Crouse integral,serum index level of the two groups were compared,and the safety of treatment was analyzed.[Results]The total effective rate of treatment in the observation group was significantly higher than that in the control group at 3 months and 6 months(P<0.05).After 6 months of treatment,IMT and Crouse scores in both groups were significantly decreased,and the two indexes in the observation group were significantly lower than those in the control group(P<0.05).After 6 months of treatment,the levels of triglyceride(TG),serum total cholesterol(TC)and low-density lipoprotein cholesterol(LDL-C)in the observation group were significantly lower than those in the control group(P<0.05).After 6 months of treatment,routine blood,urine,liver and kidney functions,electrocardiogram and other examinations were performed on the patients in the two groups,and no significant changes were observed.No serious adverse reactions occurred in the two groups during the treatment.[Conclusion]Banxia Baizhu Tianma Decoction combined with Western medicine in the treatment of carotid atherosclerosis with wind-phlegm blocked collaterals syndrome is effective and safe,and worthy of clinical promotion.
