摘要
目的分析不孕患者腹腔镜子宫肌瘤剔除术后妊娠的影响因素。方法回顾性分析2018年1月~2019年6月在我院接受腹腔镜子宫肌瘤剔除术的76例<35岁不孕患者临床资料,根据术后妊娠情况分组,其中妊娠组40例,非妊娠组36例。比较两组体重、不孕史、肌瘤类型、肌瘤数量、粘连程度、肿瘤最大直径、肌瘤是否穿透内膜、术前宫腔形态。分析影响不孕患者腹腔镜子宫肌瘤剔除术后妊娠的相关因素。结果两组体重、肌瘤类型、肌瘤最大直径及肌瘤是否穿透内膜对比,差异无统计学意义(P>0.05);两组不孕史、肌瘤数量、粘连程度以及宫腔形态比较,差异有统计学意义(P<0.05);经非条件多项Logistic回归分析显示,不孕史、肌瘤数量、粘连程度以及宫腔形态均是影响<35岁不孕患者腹腔镜子宫肌瘤剔除术后妊娠的危险因素(OR>1,P<0.05)。结论不孕史、肌瘤数量、粘连程度以及宫腔形态均可能影响<35岁不孕患者腹腔镜子宫肌瘤剔除术后妊娠情况,临床应根据上述因素决定手术方式。
Objective To analyze the factors affecting pregnancy after laparoscopic myomectomy in infertile patients.Methods A retrospective analysis of the clinical data of 76 infertile patients under 35 years of age who underwent laparoscopic myomectomy in our hospital from January 2018 to June 2019 was divided into groups according to the postoperative pregnancy status.Among them,40 cases were in the pregnant group,non-there were 36 cases in the pregnancy group.The weight,history of infertility,fibroids type,number of fibroids,degree of adhesion,maximum tumor diameter,whether the fibroids penetrate the endometrium,and the shape of the uterine cavity were compared between the two groups.To analyze the related factors affecting pregnancy after laparoscopic myomectomy in infertile patients.Results There was no significant difference in weight,fibroids type,maximum diameter of fibroids and whether fibroids penetrated the endometrium between the two groups(P>0.05);comparison of infertility history,number of fibroids,degree of adhesion,and uterine cavity morphology between the two groups,the difference was statistically significant(P<0.05);after unconditional multiple logistic regression analysis,the history of infertility,the number of fibroids,the degree of adhesion,and the shape of the uterine cavity all affect the laparoscopic uterine fibroids in infertility patients<35 years old the risk factors of postoperative pregnancy were eliminated(OR>1,P<0.05).Conclusion The history of infertility,the number of fibroids,the degree of adhesion,and the shape of the uterine cavity might affect the pregnancy of infertile patients under 35 years old after laparoscopic myomectomy.The clinical operation should be determined based on the above factors.
