添加ω-3多不饱和脂肪酸对结肠癌术后化疗患者营养指标、免疫功能的影响 被引量:4

Effects of adding omega-3 polyunsaturated fatty acid on nutritional indicators and immune function of patients undergoing chemotherapy after colon cancer surgery

摘要 目的探讨结肠癌根治术后化疗患者肠外营养添加ω-3多不饱和脂肪酸(ω-3PUFA)对患者术后康复的影响。方法依据治疗方法将120例结肠癌根治术术后患者分为研究组和对照组,每组60例。两组患者术后均给予奥沙利铂+亚叶酸钙+5-氟尿嘧啶化疗,研究组患者再此基础上添加ω-3PUFA。干预前后,比较两组患者的营养学指标[血清总蛋白(TP)、白蛋白(ALB)、转铁蛋白(TF)、血红蛋白(Hb)]、免疫功能指标[包括免疫球蛋白(IgA、IgG、IgM)和外周血T淋巴细胞(CD3+、CD4+、CD8+)]、卡氏功能状态(KPS)评分。结果干预前,两组患者营养学指标、免疫功能指标和KPS评分比较,差异均无统计学意义(P﹥0.05)。干预后,两组患者TP、ALB、Hb水平均明显高于本组干预前,TF水平明显低于本组干预前,且研究组患者的TP、ALB、Hb水平均明显高于对照组患者,研究组患者TF水平明显低于对照组患者,差异均有统计学意义(P﹤0.05)。干预后,两组患者IgG、IgM、CD3+、CD4+水平均高于本组干预前,CD8+水平低于本组干预前,且研究组患者IgG、CD3+、CD4+水平高于对照组患者,差异有统计学意义(P﹤0.05)。干预后,研究组患者的KPS评分明显高于对照组患者,且两组患者的KPS评分均高于本组干预前,差异均有统计学意义(P﹤0.01)。结论结肠癌根治术后化疗患者肠外营养添加ω-3PUFA有利于改善患者的免疫功能,并进一步提升患者的营养水平。 Objective To explore the effect of parenteral nutrition supplementation with omega-3 polyunsaturated fatty acid(ω-3PUFA)on postoperative rehabilitation of patients undergoing chemotherapy after radical resection of colon cancer.Method 120 colon cancer patients who received chemotherapy after surgery were divided into experimental group and control group each with 60 cases.Both groups received oxaliplatin+calcium folinate+5-fluorouracil chemotherapy after surgery.Before and after the intervention,the nutritional indicators[serum total protein(TP),albumin(ALB),transferrin(TF),hemoglobin(Hb)],immune function indicators[including immunoglobulin(IgA,IgG,IgM)and peripheral blood T lymphocytes(CD3+,CD4+,CD8+)],Karfsky functional status(KPS)score.Result Before the intervention,there was no significant difference in the measured values of TP,ALB,TF,and Hb between the two groups(P>0.05).After the intervention,the TP,ALB,and Hb levels of the two groups of patients were higher than those of the group before the intervention,and the TF levels were lower than those of the group before the intervention.The TP,ALB,and Hb levels of the study group were higher than those of the control group,and the TF level was low in the control group,the differences were statistically significant(P<0.05).After the intervention,the IgG,IgM,CD3+,and CD4+levels of the two groups of patients were higher than those before the intervention,and the CD8+levels were lower than those before the intervention,and the IgG,CD3+,and CD4+levels of the study group were higher than those of the control group.There is statistical significance(P<0.05).After the intervention,the KPS scores of the study group were significantly higher than those of the control group,and the KPS scores of the two groups were higher than those of the group before the intervention,the difference was statistically significant(P<0.01).Conclusion The addition ofω-3PUFA to parenteral nutrition for chemotherapy patients after colon cancer radical surgery is beneficial to improve the immune function of patients and further improve the nutritional level of patients.
