摘要
目的探讨Toll样受体4/核转录因子-κB(TLR4/NF-κB)信号通路介导的炎症反应在热毒宁注射液联合阿莫西林治疗急性扁桃体炎中的作用和意义。方法选取就诊的100例急性化脓性扁桃体炎患者,随机分为2组,观察组患者给予阿莫西林治疗,试验组患者给予热毒宁注射液联合阿莫西林治疗,选取同期50例健康体检者作为对照组。检测患者的治疗疗效和症状恢复时间;采用酶联免疫吸附法(Elisa)检测患者治疗前后血清中肿瘤坏死因子-α(TNF-α)、白细胞介素-Iβ(IL-Iβ)、干扰素-γ(IFN-γ)、白细胞介素-4(IL-4)、白细胞介素-6(IL-6)、白细胞介素-10(IL-10)、TLR4和NF-κB的含量表达;检测患者治疗前后血清中粒细胞集落刺激因子(G-CSF)、细胞毒性T淋巴细胞相关蛋白-4(CTLA-4)、降钙素原(PCT)和C-反应蛋白(CRP)含量。结果试验组治疗总有效率(98.00%,49/50)明显高于观察组(84.00%,42/50)。试验组患者治疗后的临床症状、体温恢复时间、脓点恢复时间、咽痛恢复时间和扁桃体红肿恢复时间均显著短于观察组(P<0.05)。与观察组比较,治疗后试验组患者血清内IL-4和IL-6的含量显著降低,IFN-γ和IL-10含量显著升高,TNF-α、IL-Iβ、TLR4和NF-κB的含量显著降低,G-CSF、CTLA-4、PCT和CRP的含量显著降低,差异具有统计学意义(P<0.05)。结论热毒宁注射液联合阿莫西林治疗急性扁桃体炎可有效缓解患者的临床症状,临床疗效较好;TLR4/NF-κB信号通路介导的炎症反应在治疗过程中起重要作用。
Objective To investigate the role and significance of inflammatory response mediated by Toll-like receptor 4/nuclear factor-κB(TLR4/NF-κB)signal pathway in the treatment of acute tonsillitis with Reduning Injections and amoxicillin.Methods 100 patients with hyperacute suppurative tonsillitis were randomly divided into 2 groups.The patients in the observation group were treated with amoxicillin,and the patients in the experimental group were treated with Reduning Injections combined with amoxicillin.50 healthy subjects in the same period were selected as the control group.The therapeutic effect and symptom recovery time of the patients were measured,and the expressions of tumor necrosis factor-α(TNF-α),interleukin-Iβ(IL-Iβ),interferon-γ(IFN-γ),interleukin-4(IL-4),interleukin-6(IL-6),interleukin-10(IL-10),TLR4 and NF-κB in serum before and after treatment were detected by enzyme-linked immunosorbent assay(Elisa).The serum levels of granulocyte colony stimulating factor(G-CSF),cytotoxic T lymphocyte associated protein 4(CTLA-4),procalcitonin(PCT)and C-reactive protein(CRP)were measured before and after treatment.Results The total effective rate of the experimental group(98.00%,49/50)was significantly higher than that of the observation group(84.00%,42/50).After treatment,the clinical symptoms,the recovery time of body temperature,the recovery time of purulent spot,the recovery time of sore throat and the recovery time of redness and swelling of tonsil of the experimental group were significantly shorter than those of the observation group(P<0.05).Compared with the observation group,the expression of IL-4 and IL-6 decreased significantly,the content of IFN-γand IL-10 increased significantly,the expression of TNF-α,IL-Iβ,TLR4 and NF-κB decreased significantly,and the expression of G-CSF,CTLA-4,PCT and CRP decreased significantly in the experimental group after treatment(P<0.05).Conclusion Reduning Injections combined with amoxicillin in the treatment of acute tonsillitis can effectively relieve the clinical symptoms of patients with acute tonsillitis,and the inflammatory reaction mediated by TLR4/NF-κB signal pathway plays an important role in the treatment.
