摘要
目的探讨磁共振弥散加权成像(MRI-DWI)信号强度值联合血清甲胎蛋白(AFP)、癌抗原125(CA125)在判断小肝癌分化程度中的应用价值。方法前瞻性选择108例小肝癌患者为研究对象,根据分化程度分为高分化组(n=47)、中分化组(n=35)、低分化组(n=26),检测并比较各组MRI-DWI影像学特点及血清AFP、CA125水平,分析108例小肝癌患者DWI信号强度值与血清AFP、CA125的相关性。结果中分化组MRI-DWI信号强度值、边缘模糊比例、包膜形成比例、少血供比例显著高于高分化组(P<0.05);低分化组MRI-DWI信号强度值、边缘模糊比例、包膜形成比例、少血供比例显著高于中分化组(P<0.05)。中分化组血清AFP、CA125水平显著高于高分化组(P<0.05);低分化组血清AFP、CA125水平显著高于中分化组(P<0.05)。DWI信号强度值及血清AFP、CA125与小肝癌分化程度均呈负相关(P<0.05)。结论MRI-DWI信号强度值及血清AFP、CA125水平随小肝癌患者肿瘤组织分化程度的降低而升高,MRI-DWI信号强度值联合血清AFP、CA125对小肝癌分化程度的判断有一定指导意义。
Objective To investigate application effect of magnetic resonance imaging diffusion weighted imaging(MRI-DWI)signal intensity value combined with serum alpha-fetoprotein(AFP)and cancer antigen 125(CA125)detection in the judgment of small hepatocellular carcinoma with different differentiation degree.Methods 108 patients with small hepatocellular carcinoma were prospectively selected.Patients were divided into 3 groups according to the differentiate degree,highly differentiated group(n=47),moderately differentiated group(n=35),and poorly differentiated group(n=26).The imaging characteristics of MR-DWI and the levels of serum AFP and CA125 in each group were compared.The correlation between DWI signal intensity and serum AFP and CA125 in patients with small hepatocellular carcinoma was analyzed.Results Among three groups,the intensity of MRI-DWI signal,the proportion of blurred edge,capsule formation and oligovascular supply were highest in the poorly differentiated group,followed by moderately differentiated group and highly differentiated group,with statistic difference(P<0.05).The levels of serum AFP and CA125 were highest in the poorly differentiated group,followed by moderately differentiated group and highly differentiated group,with statistic difference(P<0.05).There was a negative correlation between DWI signal intensity,serum AFP and CA125 in 108 patients with small hepatocellular carcinoma at different grades(P<0.05).Conclusion The intensity of MRI-DWI signal and the levels of serum AFP and CA125 increase with the decrease of the differentiation degree of small hepatocellular carcinoma.The combination of the intensity of MRI-DWI signal and serum AFP and CA125 has certain guiding significance in judging small liver cancer with different differentiation degree.
