摘要
目的分析湘潭市中心医院门诊2016—2018年抗精神病药物使用情况。方法选取湘潭市中心医院门诊2016—2018年抗精神病药物处方1352张,分析患者性别比例、年龄、疾病种类、处方金额、抗精神病药物使用情况〔使用频率、用药频度(DDDs)及药物利用指数(DUI)〕及不合理用药情况。结果所有患者中男女比例为0.79∶1,>18岁的患者占比较高。1352例患者中共涉及18个病种,占比前3位的病种依次是抑郁症、焦虑症、精神分裂症。单张处方金额>200元的处方占56.06%。所有处方中出现频率较高的药物依次为盐酸舍曲林片、氟哌噻吨美利曲辛片、盐酸文拉法辛缓释胶囊,三者占抗精神病药物处方总数的67.29%。盐酸舍曲林片DDDs最高,其次是盐酸文拉法辛缓释胶囊,第三位是盐酸帕罗西汀片。9种药品的DUI值均小于或接近1。处方合格率为98.67%。结论本院门诊抗精神病药物使用基本合理,但仍存在个别药物使用频次高等不合理情况,需加强管控,进一步提高其使用合理性。
Objective To analyze the use of antipsychotic drugs in outpatient department of Xiangtan Central Hospital from 2016 to 2018.Methods 1352 prescriptions of antipsychotic drugs in outpatient department of Xiangtan Central Hospital from 2016 to 2018 were selected,the sex ratio,age,disease classification,prescription amount,antipsychotic drug use(frequency of use,DDDs and DUI)and irrational drug use were analyzed.Results The ratio of male to female in all patients was 0.79∶1,and the proportion of patients over 18 years old was relatively high.A total of 18 diseases were involved in 1352 patients,and the top three diseases were depression,anxiety and schizophrenia.56.06%of the prescriptions with a single prescription amount higher than that in 200 yuan.Among all prescriptions,sertraline hydrochloride tablets,flupentixol melitracen tablets and venlafaxine hydrochloride sustained-release capsules appeared frequently,which accounted for 67.29%of the total prescriptions of antipsychotics.The DDDs of sertraline hydrochloride tablets was the highest,followed by venlafaxine hydrochloride sustained-release capsules,and paroxetine hydrochloride tablets were the third.The DUI values of 9 drugs were less than or close to 1.The qualified rate of prescription was 98.67%.Conclusion The use of antipsychotic drugs in outpatient department of our hospital is basically reasonable,but there are still unreasonable situations such as high frequency of use of individual drugs,so it is necessary to strengthen control and further improve the rationality of their use.
