摘要
目的探讨不同的宫颈细胞蜡块制作方法对制片质量的影响,以探索宫颈细胞蜡块制作较为理想的方法为宫颈细胞病理诊断提供帮助。方法收集广西医科大学第四附属医院病理科2017年10月至2018年6月宫颈液基细胞学检测的剩余标本45例,以直接取细胞沉渣法、琼脂包裹法、试剂盒制备法各收集15例细胞沉渣。采用常规石蜡切片HE染色,对三种细胞蜡块制作方法及效果进行对比分析。结果直接取细胞沉渣法取得的细胞量相对较少,制备时间长,切片内细胞片状聚集或堆叠;琼脂包裹法和试剂盒制备法取得的细胞沉渣较多,前者制备时间较长,切片内细胞平铺,较分散,可见淡蓝色琼脂背景;后者用时短,细胞平铺且集中,背景干净。结论直接取细胞沉渣法操作简单,成本低,但制备时间长,标本利用率不高;琼脂包裹法成本较低,制片效果良好,标本利用率较高,但操作时间较长且操作步骤较繁琐;试剂盒制备法操作简单,用时短,标本利用率高,制片效果好,但其成本较高。对于基层医院,可推荐采用琼脂包裹法,成本低,效果较好;对于经济条件较好的地区以及细胞量少的标本,试剂盒制备法可提高标本利用率,节约工作时间,是一种较为理想的细胞蜡块制备法。
Objective To compare the different methods of making cervical cell wax blocks and explore their influence on the quality of the slides,in order to explore an ideal method for making cervical cell wax blocks,and to provide help for pathological diagnosis of cervical cell.Methods The remaining 45 specimens of cervical fluid-based cytology from the Fourth Affiliated Hospital of Guangxi Medical University from October 2017 to June 2018 were collected.Three methods including cell sediment method,agar-wrap method,and kit preparation method were used to collect 15 cell sediment samples.Conventional paraffin sections were stained with HE,and the preparation methods and effects of three kinds of cell wax blocks were compared and analyzed.Results The amount of cells obtained by direct cell sediment method was small,the preparation time was long,and the cells in the slices were aggregated or stacked in flakes.There were more cell sediment obtained by agar wrapping method and kit preparation method.The former took a long time to prepare,and the cells in the slices were tiled and dispersed,showing a light blue agar background.The latter took a short time,the cells were tiled and concentrated,and the background was clean.Conclusion The method of directly taking the cell sediment method is simple,with low cost,long preparation time,and low sample utilization rate;the agar-wrapped method has low cost,good preparation effect,long operation time,and high sample utilization rate,but its operation steps are cumbersome;the kit preparation method is simple to operate,has a short use time and high sample utilization rate,but its cost is high.For primary hospitals,the agar-wrapped method can be recommended because of its low cost and good results.For areas with better economic conditions and specimens with fewer cells,a kit preparation method is recommended to improve work efficiency.
