摘要
目的探讨老年女性原发性开角型青光眼(POAG)的影响因素。方法随机选择我院眼科2016年11月至2019年8月确诊的老年女性POAG病人30例(观察组),选择门诊同期老年女性非POAG病人90例作为对照组。以可能影响青光眼发病的相关因素包括眼压、高血压、DM、心血管病史、青光眼家族史、吸烟、饮酒、高度近视、月经初潮年龄、绝经年龄和激素替代治疗病史等作为观察指标进行单因素分析,单因素分析有意义者进一步行非条件二分类Logistic多因素回归分析。结果单因素分析显示,眼压、高血压、青光眼家族史、高度近视、绝经年龄≥51岁以及激素替代治疗史与老年女性POAG发病有关。Logistic多因素回归分析显示,高眼压、高血压、青光眼家族史、高度近视是老年女性POAG的独立危险因素;而绝经年龄≥51岁以及激素替代治疗史是老年女性POAG的保护性因素。结论老年女性原发性青光眼并不罕见,高眼压、高血压、青光眼家族史以及高度近视是老年女性POAG发生的独立影响因子,而较高的女性性激素水平可能对POAG的发生有保护作用。
Objective To investigate the influencing factors of primary open-angle glaucoma(POAG)in the elderly women.Methods A total of thirty elderly female patients with POAG in our hospital from November 2016 to August 2019 were randomly selected as the observation group.In addition,90 cases of elderly female patients without POAG at the outpatient during the same period were selected as the control group.Relevant factors that may affect the onset of POAG including intraocular pressure,hypertension,diabetes,history of cardiovascular disease,family history of glaucoma,smoking,alcohol consumption,high myopia,age of menarche and age of menopause,history of hormone replacement therapy were analyzed.Results Univariate analysis showed that intraocular pressure,hypertension,family history of glaucoma,high myopia,menopausal age(>51 years)and history of hormone replacement therapy were associated with POAG in the elderly women.Logistic multivariate regression analysis showed that high intraocular pressure,hypertension,family history of glaucoma and high myopia were independent risk factors for POAG in the elderly women.However,menopausal age(>51 years)and history of hormone replacement therapy were protective factors for POAG in elderly women.Conclusions POAG is common in the elderly women.High intraocular pressure,hypertension,family history of glaucoma and high myopia are independent influencing factors,and higher levels of female sex hormones may protect the occurrence of POAG.
