摘要
目的探讨帕罗西汀治疗恶性血液病伴发焦虑、抑郁症状的临床效果。方法选择2016年2月至2019年10月我院血液科收治的150例恶性血液病伴发焦虑、抑郁症状患者作为研究对象,依据随机数字表法将其分为对照组和观察组,每组75例。对照组实施常规治疗,观察组在化疗基础上加用帕罗西汀。比较两组的焦虑、抑郁评分、免疫功能指标及生活质量评分。结果治疗前,两组HAMA、HAMD评分比较,差异无统计学意义(P>0.05);治疗后,观察组的HAMA、HAMD评分显著低于治疗前及对照组,差异具有统计学意义(P<0.05)。治疗前,两组的IgM、IgG、IgA、CD3^+、CD4^+/CD8^+比较,差异无统计学意义(P>0.05);治疗后,观察组的IgM、IgG、IgA、CD3^+、CD4^+/CD8^+高于治疗前及对照组,差异具有统计学意义(P<0.05)。治疗前,两组的生活质量各项评分比较,差异无统计学意义(P>0.05);治疗后,两组的生活质量各项评分均提高,且观察组高于对照组,差异具有统计学意义(P<0.05)。结论帕罗西汀用于恶性血液病伴有焦虑、抑郁症状患者中可减轻焦虑、抑郁症状,还可改善免疫功能,有利于提高患者的生活质量,值得临床推广应用。
Objective To investigate the clinical effect of paroxetine in the treatment of malignant hematopathy with anxiety and depression.Methods From February 2016 to October 2019,150 malignant hematopathy patients with anxiety and depression symptoms admitted in the hematology department of our hospital were selected as study objects and divided into control group and observation group according to the random number table method,with 75 cases in each group.The control group received routine treatment,and the observation group added paroxetine on the basis of chemotherapy.The anxiety scores,depression scores,immune function indexes and quality of life scores were compared between the two groups.Results Before treatment,there were no significant differences in HAMA and HAMD scores between the two groups(P>0.05);after treatment,the scores of HAMA and HAMD in the observation group were significantly lower than those before treatment and in the control group,and the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in IgM,IgG,IgA,CD3^+and CD4^+/CD8+between the two groups(P>0.05);after treatment,the IgM,IgG,IgA,CD3^+and CD4^+/CD8^+in the observation group were higher than those before treatment and in the control group,and the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in the scores of quality of life between the two groups(P>0.05);after treatment,the scores of quality of life of the two groups improved,those in the observation group were higher than the control group,and the differences were statistically significant(P<0.05).Conclusion Paroxetine for malignant hematopathy patients with anxiety and depression symptoms can reduce anxiety and depression symptoms,improve the immune function and quality of life,which is worthy of clinical popularization and application.
