摘要
目的:探讨MRI诊断鼻腔鼻窦恶性黑色素瘤的价值及结果。方法:选取笔者所在医院2014年1月-2019年6月收治的鼻腔鼻窦恶性黑色素瘤患者35例,患者均接受手术病理学检查,分析患者手术前MRI诊断结果与病理学检查的一致性。结果:手术病理学检查显示,鼻腔内肿瘤24例,上额窦内肿瘤6例,筛窦内肿瘤3例,额窦内肿瘤2例。29例黑色素型,6例非黑色素型。MRI诊断检查结果显示,肿瘤形态不规则,呈膨胀性生长,鼻腔内肿瘤23例,上额窦内肿瘤5例,筛窦内肿瘤2例,额窦内肿瘤2例。病灶部位诊断结果一致性为91.43%。29例典型黑色素瘤患者,T1WI呈高信号,T2WI低信号特征。6例肿瘤内无黑色素患者,MRI信号呈现为T1WI等信号、低信号,T2WI稍高或高信号的不典型表现。MRI影像诊断中,29例黑色素型,6例非黑色素型,诊断结果准确率达100%。结论:以MRI检查方式能够较准确的确定鼻腔鼻窦恶性黑色素瘤病灶部位,且MRI诊断下对典型恶性黑色素瘤具有特异性,能够为临床诊断提供有效依据。
Objective:To explore the value and result of MRI in the diagnosis of malignant melanoma of the nasal cavity and sinuses.Method:A total of 35 patients with malignant melanoma of the nasal cavity and sinuses admitted in our hospital from January 2014 to June 2019 were selected.All patients underwent surgical pathology examination,and the consistency of MRI diagnosis results before surgery and pathological examination was analyzed.Result:Surgical pathological examination showed that there were 24 cases of tumors in nasal cavity,6 cases of tumors in superior frontal sinus,3 cases of tumors in ethmoid sinus and 2 cases of tumors in frontal sinus.There were 29 cases of melanoma type and 6 cases of non-melanoma type.The results of MRI diagnosis showed that there were 23 cases of tumors in nasal cavity,5 cases of tumors in superior frontal sinus,2 cases of tumors in ethmoid sinus and 2 cases of tumors in frontal sinus.The consistency of lesion site diagnosis was 91.43%.In 29 patients with typical melanoma showed high signal intensity on T1WI and low signal intensity on T2WI.In 6 patients with non-melanoma,MRI signal showed the atypical manifestations of equisignal and low signal on T1WI and slightly high signal on T2WI.In MRI diagnosis,29 cases were melanoma type and 6 cases were non-melanoma type,and the diagnostic accuracy reached 100%.Conclusion:MRI examination can more accurately determine the lesion site of malignant melanoma of the nasal cavity and sinuses,and MRI diagnosis is specific to typical malignant melanoma,which can provide an effective basis for clinical diagnosis.
