摘要
目的:了解北京中医药大学东方医院(以下简称"我院")门急诊中药饮片临床使用情况,分析不合理处方存在的问题,以促进中药饮片的合理用药。方法:从2019年我院门急诊中药饮片处方中随机抽取2650张,根据相关法定文件将不合理处方分为不规范处方、用药不适宜处方和常超处方(用药味数偏多、金额偏大),分别进行统计分析。结果:2650张门急诊中药饮片处方中,不规范处方166张(占抽取处方数的6.26%),主要涉及开具处方临床诊断书写不全、有毒中药超剂量使用未签字以及未标注调剂、特殊煎煮要求等;用药不适宜处方51张(占抽取处方数的1.92%),主要涉及处方中用法不适宜、处方中存在配伍禁忌未签字;单张处方中药味数为1~34味,平均13.73味,其中>30味药处方共6张(占抽取处方数的0.23%);单剂药平均金额为39.52元,其中单剂药金额>200元的处方共9张(占抽取处方数的0.34%)。结论:我院中药饮片处方合理情况良好,但仍存在一些问题,包括诊断书写不全、有毒中药超量使用等。需加强医师中医知识的培训,提升药师队伍业务水平,完善中药处方点评相关制度,以促进中药饮片的合理使用。
OBJECTIVE:To investigate the application of traditional Chinese medicine decoction pieces in outpatient and emergency departments in Dongfang Hospital of Beijing University of Chinese Medicine(hereinafter referred to as"our hospital"),and to analyze the problems in irrational prescriptions,so as to promote rational medication of traditional Chinese medicine decoction pieces.METHODS:Totally 2650 prescriptions were randomly extracted from outpatient and emergency prescriptions of traditional Chinese medicine decoction pieces in our hospital in 2019,of which the irrational prescriptions were classified into nonstandard prescriptions,inappropriate prescriptions and abnormal prescriptions(too much categories of traditional Chinese medicine and too large amount of prescription cost)according to related statutory instruments.RESULTS:Among the 2650 prescriptions of traditional Chinese medicine decoction pieces,166 were nonstandard prescriptions(6.26%),mainly including incomplete writing of clinical diagnosis on prescription,overdose application of toxic drugs without signature and unmarked adjustment,special decoction requirements,etc;51 were appropriate prescriptions(1.92%),mainly including inappropriate usage and incompatibility without signature;the number of Chinese medicine flavors in a single prescription was 1 to 34,with an average of 13.73,there were 6 prescriptions were>30 flavors(0.23%);the average amount of a single prescription was 39.52 yuan,of which 9 prescriptions were>200 yuan(0.34%).CONCLUSIONS:Prescriptions of traditional Chinese medicine decoction pieces in our hospital is in good condition,while there are still some problems,including incomplete writing of clinical diagnosis and overdose of toxic traditional Chinese medicine.It is necessary to strengthen the training on traditional Chinese medicine knowledge,improve the professional level of pharmacists,and improve relevant system of traditional Chinese medicine prescription review to promote rational application of traditional Chinese medicine decoction pieces.
