摘要
目的分析恩替卡韦对慢性乙型肝炎患者Treg/Th17细胞平衡及相关细胞因子水平的影响。方法以2016年8月~2019年8月石河子大学医学院第一附属医院感染性疾病科门诊收治的200例慢性乙型肝炎患者作为研究对象,根据患者意愿分为对照组(n=90)、观察组(n=110),对照组给予阿德福韦酯治疗,观察组予以恩替卡韦治疗,评定2组患者的ALT复常率、HBV-DNA转阴率、HBeAg转阴率、Treg细胞、Th17细胞表达率、外周血浆细胞因子IL-35、IL-17浓度。结果治疗前,2组患者的Treg细胞、Th17细胞表达率、外周血浆细胞因子IL-35、IL-17浓度无显著差异,P>0.05;治疗后,观察组相较于对照组患者的Treg细胞、Th17细胞表达率、外周血浆细胞因子IL-35、IL-17浓度改善显著,P<0.05;其二观察组相较于对照组患者的ALT复常率、HBV-DNA转阴率较高,P<0.05;HBeAg转阴率无差异,P>0.05。结论慢性乙型肝炎患者恩替卡韦治疗具有积极的意义,可在易于改善患者的Treg细胞、Th17细胞表达率、外周血浆细胞因子IL-35、IL-17浓度,提升ALT复常率、HBV-DNA转阴率,值得临床深究与采纳。
Objective To analyze the effect of entecavir on Treg/Th17 cell balance and related cytokine levels in patients with chronic hepatitis B.Methods 200 patients with chronic hepatitis B admitted to the department of infectious diseases at the First Affiliated Hospital of Shihezi University from August 2016 to August 2019 were used as the research objects,and the patients were divided into control groups(n=90)and observation group(n=110)according to their wishes.The control group was treated with adefovir dipivoxil.The observation group was treated with entecavir.ALT normalization rate,HBV-DNA negative rate,HBe Ag negative rate,Treg cells,Th17 cell expression rate,peripheral plasma cytokines IL-35 and IL-17 concentration were detected and evaluated among the patients.Results Before treatment,there were no significant differences in Treg cells,Th17 cell expression rates,and peripheral plasma cytokines IL-35,IL-17 concentrations in the two groups of patients,(P>0.05);After treatment,Th17 cell expression rate,peripheral plasma cytokines IL-35,IL-17 concentrations in the observation group were significantly improved compared with the control group,(P<0.05);Patients of the observation group had higher ALT recovery rate and HBV-DNA negative rate compared with the control group,(P<0.05);There was no difference in HBe Ag negative rate,(P>0.05).Conclusions Entecavir treatment in patients with chronic hepatitis B has positive significance.It can easily improve the Treg cells,Th17 cell expression rate,peripheral plasma cytokines IL-35 and IL-17 concentration,increase ALT normalization rate and HBV-DNA negative rate,so it is worthy of clinical promotion.
