摘要
目的研究多囊卵巢综合征(PCOS)患者雄激素异常表达与胰岛素抵抗的相关性。方法抽取2019年2月至2020年2月于商丘市第一人民医院接受治疗的108例PCOS患者设为观察组,另抽取同期健康育龄女性100例作为对照组,检测两组受试者性激素水平[促黄体生成素(LH)、睾酮(T)、雌二醇(E2)、卵泡刺激素(FSH)]及胰岛素相关指标[空腹血糖(FBG)、空腹胰岛素( FINS) 、胰岛素抵抗指数(HOMA-IR)],并分析性激素水平与胰岛素抵抗间的关系。结果观察组LH、T水平高于对照组[(9.63±1.31)IU/L比(5.54±1.71)IU/L,(1.25±0.16)ng/ml比(0.42±0.09)ng/ml],E2、FSH低于对照组[(44.28±5.02)pg/ml比(63.84±8.82)pg/ml,(4.24±1.71)IU/L比(8.76±2.89)IU/L],P<0.05;观察组FBG、 FINS水平及HOMA-IR值高于对照组[(4.95±0.93)mmol/L比(4.47±0.54)mmol/L,(15.34±4.71)U/L比(8.16±1.89)U/L,5.37±1.45比2.08±0.73],P<0.05。E2、FSH、LH与HOMA-IR无相关性(P>0.05),T与HOMA-IR呈高度正相关(P<0.05,r=0.572)。结论 PCOS患者存在一定程度性激素分泌紊乱及胰岛素抵抗现象,且雄激素T水平与胰岛素抵抗具有一定相关性。
Objective To investigate the correlation between abnormal androgen expression and insulin resistance in patients with polycystic ovary syndrome(PCOS).Methods A total of 108 PCOS patients treated in the First People’s Hospital of Shangqiu from February 2019 to February 2020 were selected as the observation group,and 100 healthy women of childbearing age were selected as the control group.The indexes of the two groups were detected,including sex hormone levels,such as luteinizing hormone(LH),testosterone(T),estradiol(E2),follicle stimulating hormone(FSH),and insulin related indicators,such as fasting blood glucose(FBG),fasting insulin(FINS),homeostatic model assessment for insulin resistance(HOMA-IR).The relationship between sex hormone levels and insulin resistance was analyzed.Results Levels of LH and T of observation group were(9.63±1.31)IU/L and(1.25±0.16)ng/ml,respectively,significantly higher than the(5.54±1.71)IU/L and(0.42±0.09)ng/ml of control group(P<0.05).And levels of E2 and FSH of observation group were(44.28±5.02)pg/ml and(4.24±1.71)IU/L,significantly lower than the(63.84±8.82)pg/ml and(8.76±2.89)IU/L of control group(P<0.05).Levels of FBG,FINS and HOMA-IR values of observation group were significantly higher than those of control group,the comparisons were(4.95±0.93)mmol/L vs.(4.47±0.54)mmol/L,(15.34±4.71)U/L vs.(8.16±1.89)U/L,5.37±1.45 vs.2.08±0.73,respectively,P<0.05.E2,FSH and LH were not correlated with HOMA-IR(P>0.05),while T was highly positively correlated with HOMA-IR(P<0.05,r=0.572).Conclusions PCOS patients have a certain degree of sex hormone secretion disorder and insulin resistance,and there is a certain correlation between androgen T level and insulin resistance.
