摘要
目的探讨临床护理路径(CNP)在神经科痉挛性瘫痪患者改良外周静脉置入中心静脉导管(PICC)置管中的效果。方法选取2019年1~12月本院神经科收治的痉挛性瘫痪患者102例,应用随机数字表法将患者分为研究组(n=51)与对照组(n=51)。对照组采用B超引导下PICC常规护理路径,研究组采用B超引导下改良PICC置管CNP。比较两组患者的一次穿刺成功率、置管时间、穿刺点的愈合情况、一周内换药次数及导管留置期间的并发症发生率。结果研究组一次穿刺成功率高于对照组,置管时间短于对照组,差异有统计学意义(P<0.05)。研究组穿刺点愈合良好率高于对照组,差异有统计学意义(P<0.05)。穿刺后一周内换药次数少于对照组,差异有统计学意义(P<0.05)。研究组PICC置管后并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论在神经科痉挛性瘫痪患者应用改良PICC置管CNP,能有效提高一次穿刺成功率,利于穿刺点的愈合,降低了PICC并发症的发生率,提高了护理质量。
Objective To explore the effect of clinical nursing pathways(CNP)in neurological spastic paralysis patients with improved peripherally inserted central catheter(PICC).Methods From January to December 2019,a total of 102 patients with spastic paralysis admitted to the department of neurology of our hospital were selected,and the patients were divided into a study group(n=51)and a control group(n=51)using a random number table method.The control group adopted the routine nursing pathway of PICC under the guidance of B-ultrasound,and the study group adopted the modified PICC catheterization CNP under the guidance of B-ultrasound.The success rate of a puncture,the catheterization time,the healing of the puncture point,the number of dressing changes in one week,and the incidence of complications during catheter indwelling were compared between the two groups.Results The success rate of a puncture in the study group was higher than that of the control group,and the catheter placement time was shorter than that of the control group,the differences were statistically significant(P<0.05).The puncture point healing rate of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).The number of dressing changes in one week after puncture was fewer than that of the control group,the difference was statistically significant(P<0.05).The incidence of complications after PICC catheterization in the study group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion The application of modified PICC catheterization CNP in neurological spastic paralysis patients can effectively improve the success rate of a puncture,facilitate the healing of the puncture point,reduce the incidence of PICC complications,and improve the quality of care.
