摘要
目的研究完全无管化与部分无管化经皮肾镜取石术(PCNL)的适应证。方法从2018年1月至2019年12月于我院行PCNL患者中选取60例入组研究,排除术中发生严重出血以及不符合无管化PCNL患者,共52例入组研究,采用随机数字表法分为对照组和观察组,每组26例。对照组行部分无管化PCNL,观察组行完全无管化PCNL。对两组患者手术情况、手术并发症发生情况、术后并发症发生情况进行评价和比较。结果观察组患者手术时间显著短于对照组,手术并发症发生率低于对照组(P<0.05);两组患者术后肾周积液、肾绞痛、尿瘘等并发症发生率比较未见显著统计学差异(P>0.05)。结论就手术效果、安全性来说,完全无管化PCNL优于部分无管化PCNL,在术前检查阶段要仔细确认患者体征,然后再进行手术方案的选择,以确保手术顺利进行。
Objective To study the indications for complete and partial tubeless percutaneous nephrolithotomy(PCNL).Methods From January 2018 to December 2019,60 patients who underwent percutaneous nephrolithotomy in our hospital were selected for enrollment in the study,and severe bleeding during the operation and patients who did not meet the tubeless PCNL were excluded,a total of 52 cases were included in the study and were divided into control group and observation group by random number table,with 26 cases in each group.The control group underwent partial tubeless PCNL,and the observation group underwent completely tubeless PCNL.The operative conditions,operative complications,and postoperative complications of the two groups were evaluated and compared.Results The operation time of the observation group was significantly shorter than that of the control group,and the incidence of surgical complications was lower than that of the control group(P<0.05);there was no significant difference in the incidence of postoperative complications such as perirenal effusion,renal colic and urinary fistula between the two groups(P>0.05).Conclusion In terms of surgical results and safety,completely ruined PCNL is better than partially ruined PCNL.The patient's physical signs must be carefully confirmed in the preoperative examination stage,and then the selection of the surgical plan is carried out to ensure the smooth progress of the operation.In terms of surgical effect and safety,completely tubeless PCNL is more excellent than partial tubeless PCNL.Carefully confirm the patient’s physical signs during the preoperative examination,then choose the surgical plan to ensure the smooth progress of the operation.
