高流量鼻导管湿化氧疗护理在儿童腺病毒肺炎中的应用研究 被引量:4

摘要 目的探讨高流量鼻导管湿化氧疗(humidified high flow nasal cannula,HHFNC)护理在儿童腺病毒肺炎中的应用效果。方法选取80例2019年1—12月腺病毒肺炎的患儿,观察组采用高流量鼻导管湿化氧疗,对照组采用普通的氧气湿化装置氧疗(氧流量≤2 L/min),观察记录两组氧疗方式过程中患儿24 h的PaO2、PaCO2、血氧饱和度、二氧化碳潴留情况、插管率、重症监护室的住院天数及家属满意度等情况。结果观察组氧疗过程中患儿24 h PaO2比对照组高,PaCO2比对照组低,血氧饱和度≤95%例数比对照组少(4例比30例),差异均有统计学意义(均P<0.05);观察组的有创气管插管率比对照组低[20%(8/40)比70%(28/40)],观察组平均重症监护住院天数较对照组短(4 d比8 d),患儿家属100%满意人数显著升高,两组比较差异均有统计学意义(均P<0.05)。结论采用高流量鼻导管湿化氧疗护理过程中,患儿PaO2显著升高,PaCO2降低,进行有创气管插管人数明显降低,重症监护住院天数缩短,患儿家属满意度大幅度提升,具有实用价值。 Objective To investigate the clinical effect of humidified high flow nasal cannula(HHFNC)nursing in children with adenovirus pneumonia.Methods In this study,80 cases of children with adenovirus pneumonia from January 2019 to December 2019 in pediatric intensive care unit(PICU)were randomly divided into control group and observation group.The observation group was treated with HHFNC,while the control group was treated with low flow continuous oxygen inhalation with ordinary oxygen humidification device(oxygen flow≤2 L/min).The following indicators were compared between the two groups,including partial pressure of oxygen(PaO2),partial pressure of carbon dioxide(PaCO2),and blood oxygen saturation in the blood gas analysis at 24 hours post treatment,the proportion of intubation and invasive ventilation,hospitalization days in PICU,and the satisfaction degree of their family members.Results PaO2 in blood gas analysis at 24 hours post treatment in the observation group was significantly higher than that in the control group,while the PaCO2 in blood gas analysis at 24 hours post treatment was significantly lower than that in the control group(both P<0.05).The proportion of blood oxygen saturation≤95%in the observation group(4/40)was significantly lower than that in the control group(30/40)(P<0.05).The proportion of invasive ventilation in the observation group(20%)was significantly lower than that in the control group(70%)(P<0.05).The average hospitalization days in PICU was 4 days in the observation group,which was significantly shorter than that in the control group(8 days)(P<0.05).The rate of 100%satisfaction of their family members in the observation group was significantly higher than that in the control group(97.5%vs.80.0%)(P<0.05).Conclusion The application of HHFNC for children with adenovirus pneumonia can significantly improve oxygenation,reduce carbon dioxide retention,reduce the proportion of invasive ventilation,shorten the length of hospitalization in PICU,increase the satisfaction degree of their family members,which is a suitable noninvasive respiratory support technology.
