摘要
目的探讨输注冷沉淀的时间对严重创伤性失血患者的影响。方法回顾性分析2018年2月至2020年2月本院的124例严重创伤性失血患者的临床资料,根据输注冷沉淀时间不同将其分为对照组(n=61)和研究组(n=63)。对照组输注第1袋红细胞悬液时间与第1单位冷沉淀输注时间间隔≥3 h,研究组输注第1袋红细胞悬液时间与第1单位冷沉淀输注时间间隔<3 h。比较两组的治疗效果。结果研究组的止血成功率为96.83%,高于对照组的85.25%(P<0.05);研究组的病死率为3.17%,低于对照组的16.39%(P<0.05)。输注后3 h,两组PT、APTT、TT均短于输注前1 h,PLT高于输注前1 h,且研究组优于对照组,差异具有统计学意义(P<0.05)。研究组24 h的平均红细胞悬液、血浆用量均少于对照组(P<0.05)。两组的并发症总发生率无显著差异(P>0.05)。结论输注第1袋红细胞悬液时间与第1单位冷沉淀输注时间间隔<3 h在严重创伤性失血患者中有利于改善凝血功能,提升止血成功率,减少输血量,降低并发症发生率及病死率。
Objective To investigate the influence of time of cryoprecipitate infusion on patients with severe traumatic blood loss.Methods The clinical data of 124 patients with severe traumatic blood loss in our hospital from February 2018 to February 2020 were retrospectively analyzed.According to the different time of cryopreservation infusion,the patients were divided into control group(n=61)and study group(n=63).In the control group,the infusion time interval between the red blood cell suspension of first bag and cryoprecipitate of the first unit was≥3 h,while that of the study group was<3 h.The therapeutic effects of the two groups were compared.Results The success rate of hemostasis in the study group was96.83%,which was higher than 85.25%in the control group(P<0.05);the fatality rate of the study group was 3.17%,which was lower than 16.39%of the control group(P<0.05).At 3 h after infusion,PT,APTT and TT of the two groups were shorter than those at 1 h before infusion,PLT was higher than that at 1 h before infusion,and those in the study group were better than the control group,and the differences were statistically significant(P<0.05).The amount of average red blood cell suspension and plasma in 24 h in the study group were less than those in the control group(P<0.05).There was no significant difference in the total incidence of complications between the two groups(P>0.05).Conclusion In patients with severe traumatic blood loss,the infusion time interval between the red blood cell suspension of first bag and cryoprecipitate of the first unit was<3 h can improve the coagulation function and the success rate of hemostasis,reduce the amount of blood transfusion and the incidence of complications and fatality.
