摘要
目的探讨前列地尔联合贝前列素钠辅助血液透析对老年慢性肾衰竭(CRF)患者的残余肾功能、炎性因子、血脂、血液黏度及营养状况的影响。方法将104例CRF患者随机分为对照组和观察组各52例。两组患者均给予血液透析治疗及口服贝前列素钠片,观察组同时给予静脉滴注前列地尔。评价两组患者临床疗效,比较两组患者治疗前后的残余肾功能、血脂水平、血液黏度、血清炎症细胞因子和核因子κB抑制蛋白α(IKB-α)水平、蛋白质能量消耗情况,记录两组患者不良反应发生情况。结果观察组总有效率高于对照组(P<0.05)。治疗后两组患者的血清肌酐、血胱抑素、24 h尿蛋白定量、总胆固醇、三酰甘油、全血低切黏度、全血高切黏度、白细胞介素6、白细胞介素8、肿瘤坏死因子α、IKB-α、主观全面评定量表评分均低于治疗前,观察组肾小球滤过率、体重、上臂肌围、小腿围高于对照组(均P<0.05)。两组不良反应发生率差异无统计学意义(P>0.05)。结论前列地尔联合贝前列素钠辅助血液透析治疗老年CRF患者具有较好的临床疗效,可以明显地改善患者的残余肾功能,降低炎症反应水平,调节血脂和血液黏度,有助于改善老年患者的营养状况,且安全性较好。
Objective To explore the effect of hemodialysis with adjuvant alprostadil and beraprost sodium on residual renal function,inflammatory factors,blood lipid,blood viscosity,and nutrition status in elderly patients with chronic renal failure(CRF).Methods A total of 104 CRF patients were randomly divided into control group and observation group,with 52 cases in each group.Both groups were given hemodialysis and oral Beraprost Sodium Tablets,and the observation group was given intravenous drip of alprostadil simultaneously.Clinical efficacy on patients was assessed in both groups,residual renal function,blood lipid level,blood viscosity,serum inflammatory cytokines and nuclear factorκB inhibitory proteinα(IKB-α)levels,and protein energy consumption were compared between the two groups before and after treatment,and the incidence of adverse reactions of the two groups was recorded.Results The total effective rate of the observation group was higher than that of the control group(P<0.05).After treatment,serum creatinine,serum cystatin,24-hour urine protein quantification,total cholesterol,triglycerides,whole blood low-shear viscosity,whole blood high-shear viscosity,interleukin 6,interleukin 8,tumor necrosis factorα,IKB-α,and the Subjective Comprehensive Evaluation Scale score in the two groups were lower than those before treatment,and glomerular filtration rate,body weight,upper arm muscle circumference,and calf circumference in the observation group were higher than those in the control group(all P<0.05).There was no statistically significant difference in incidence rate of adverse reactions between the two groups(P>0.05).Conclusion Hemodialysis with adjuvant alprostadil and beraprost sodium has a favorable clinical efficacy for the treatment of elderly patients with CRF,which can significantly improve patient′s residual renal function,reduce inflammation response level,regulate blood lipid and blood viscosity,contributing to improving nutrition status of elderly patients with favorable safety.
