摘要
目的分析老年肱骨近端骨折采用锁定接骨板治疗的远期疗效及并发症发生情况。方法选取该院2017年1月—2019年9月收治的64例老年肱骨近端骨折患者为研究对象,随机分为两组,对照组采用解剖接骨板治疗,观察组采用锁定接骨板治疗,对比两组远期疗效、肩关节活动度(ROM)并测定结果以及并发症发生情况。结果术后6个月,观察组肩关节ROM各项测定值均优于对照组,差异有统计学意义(t=15.719、30.501、20.170、25.794、P<0.05);观察组优良率为90.63%,高于对照组的68.75%(χ^2=4.730,P<0.05);观察组肱骨头坏死、肱骨头内翻畸形愈合发生率分别为3.13%、6.25%,均低于对照组的28.13%、34.38%(χ^2=7.181、7.256,P<0.05)。结论老年肱骨近端骨折采用锁定接骨板治疗的远期疗效理想,并发症发生率低。
Objective To analyze the long-term effects and complications of the locking plate for treatment of proximal humeral fractures in the elderly.Methods Sixty-four elderly patients with proximal humeral fractures admitted to the hospital from January 2017 to September 2019 were selected as the research objects.They were randomly divided into two groups.The control group was treated with an anatomical plate and the observation group was treated with a locking plate.The long-term curative effect,the range of motion(ROM)of the shoulder joint and the measurement results and complications occurred in the two groups.Results At 6 months after operation,the measured values of shoulder joint ROM in the observation group were better than those in the control group,the difference was statistically significant(t=15.719,30.501,20.170,25.794,P<0.05);the excellent and good rate of the observation group was 90.63%,which was higher than that in the control group 68.75%,the difference was statistically significant(χ^2=4.730,P<0.05);the incidence of humeral head necrosis and humeral head varus deformity healing in the observation group were 3.13%and 6.25%,respectively,which were lower than those in the control group of 28.13%and 34.38%,the difference was statistically significant(χ^2=7.181,7.256,P<0.05).Conclusion The long-term curative effect of the locking plate for the treatment of proximal humeral fractures in the elderly is ideal,and the incidence of complications is low.
