摘要
目的观察有晶状体眼后房型人工晶状体(implantable contact lens,ICL)及有晶状体眼后房散光型人工晶状体(toric implantable col1amer lens,TICL)植入术治疗中、高度近视及近视合并散光的有效性和安全性。方法回顾性分析中、高度近视患者共计30例(51眼)植入ICL/TICL后,对比观察术前及术后3月时裸眼视力、眼压、角膜内皮细胞计数情况,分析术后并发症情况。结果术后3月复诊随访时裸眼视力均较术前显著提高,术后裸眼视力与术前BCVA相同的有18眼(35.29%),高于术前BCVA 1行的12眼(23.53%),提高2行的11眼(21.57%),提高3行以上9眼(17.60%),无BCVA下降的病例。术后UCVA优于术前BCVA的患者合计20眼(39.21%),术后平均眼压(15.21±3.09)mmHg,与术前对比差异无统计学意义(t=1.200,P>0.05)。术后角膜内皮细胞密度较术前稍有减少,对比差异无统计学意义(t=1.075,P>0.05),术后均未发生眼内感染、眼底出血、黄斑水肿等并发症。1例TICL术中损伤旁中央前囊膜约0.3 mm,皮质无明显损伤,评估后植入晶体,术后3月随访患者裸眼视力保持在1.0前囊膜损伤处瓷白色,局限。1例TICL患者(一眼)术中前房填充粘弹剂后在植入人工晶体过程中发现人工晶体破损,手术终止,未予以植入TICL,术后常规滴眼液点眼,随访观察至术后3个月未发现特殊并发症,BCVA与术前相同。结论ICL/TICL对于高度近视的患者,特别是高度近视合并中高度数散光的患者具有良好的光学矫正效果。
Objective To observe the efficacy and safety of implantable contact lens(ICL)and toric implantable col1 amer lens(TICL)for correcting moderate and high myopia with or without astigmatism.Methods Retrospective analysis was performed in this study,a total of 30 patients with moderate and high myopia with or without astigmatism(51 eyes)were implanted with ICLs/TICLs.Preoperative and postoperative visual acuity,intraocular pressure(IOP),corneal endothelial cell count were contrasted,and the postoperative complications following surgery were analyzed.Results All patients got a better BCVA after the surgery 3 month after the surgery.18 eyes(35.29%)got the BCVA as good as UCVA that pre-operation,12 eyes(23.53%)got higher in 1 line than that of preoperative BCVA,and 11 eyes(21.57%)higher in 2 lines,9 eyes(17.60%)got higher in more than 3 lines.There was no difference in intraocular pressure(t=1.200,P>0.05)and corneal endothelial cell count(t=1.075,P>0.05)before and after the surgery.Complications such as macular hemorrhage,haemorrhage in ocular fundus,etc.were not found.There was one lens anterior capsule damage(0.3 mm)while the TICL surgery.Implantation TICL after Evaluation.The UCVA kept at 1.0 while 3 month after the surgery.The damage was whitened and localized.One TICL surgery has to be stoped,because of the TICL lense was fond damaged while implanting.No complication was found after the surgery,and the BCVA has not declined in the last follow up(3 month).Conclusion Implantable contact lens(ICL)and toric implantable col1 amer lens(TICL)is an elfective way to cure high myopia,especially high myopia with or without astigmatism.
