摘要
目的探讨儿童葡萄球菌性烫伤样皮肤综合征(staphylococcal scalded skin syndrome,SSSS)的临床特点及治疗效果。方法对我院2014年11月-2019年6月收治的7例SSSS患儿的临床表现、治疗措施及疗效进行回顾性分析。结果本组7例患儿均有泛发型红斑,红斑上有松弛性大疱、糜烂,尼氏征阳性,口周放射状皱襞或皲裂伴有眼部充血及脓性分泌物6例;外周血白细胞升高3例;发热2例。所有患儿均使用头孢曲松钠(罗氏芬)治疗5d^9d后痊愈出院,其中1例因皮损严重,加用甲泼尼龙抗炎治疗及局部创面护理。结论SSSS多见于3岁~6岁儿童,早期诊断、及时应用β-内酰胺类抗生素及适当的皮肤局部处理是治疗关键,严重者可加用肾上腺皮质激素,以尽早恢复。
Objective To Discussion the clinical characteristics and therapeutic effect of Staphylococcal scalded skin Syndrome(SSSS).Methods The clinical manifestations,treatment measures and efficacy of 7 SSSS children admitted to the Department of Pediatrics,Shenmu City Hospital from November 2014 to June 2019 were analyzed retrospectively.Results In this study,all the 7 children aged from 2 months to 6 years old had generalized erythema,with flaccid bullae,erosion and positive Nissl sign on the erythema,and 6 children had perioral radial folds or chaps accompanied by eye congestion and purulent secretions.3 cases of leukocyte elevation in peripheral blood;Fever 2 cases;All patients were treated with ceftriaxone sodium(Roxburne)for 5-9 days and discharged from hospital.One patient was treated with methylprednisolone anti-inflammatory treatment and local wound care due to severe skin lesions.Conclusion All the 7 cases of SSSS in this group were mostly observed in children aged 3-6 years.Early diagnosis,timely application of-lactam antibiotics and appropriate local skin treatment were the key to the treatment.In serious cases,adrenocorticosteroids can be added to recover as soon as possible.
