高频超声检查诊断甲状腺乳头状癌微钙化的价值分析 被引量:5

摘要 目的分析17 MHz高频超声诊断甲状腺乳头状癌微钙化的价值。方法选取2016年2月至2018年5月在西安市第九医院就诊的163例甲状腺乳头状癌病人为研究对象。所有病人均接受常规超声及17 MHz高频超声检查。观察并记录甲状腺乳头状癌病人甲状腺结节的部位、边界、内部回声、后方回声等。采用受试者工作特征曲线(ROC)分析17 MHz高频超声诊断甲状腺乳头状癌微钙化的价值。分析甲状腺乳头状癌微钙化与甲状腺乳头状癌病人临床病理的关系。结果病理检查证实163例甲状腺乳头状癌病人中28例伴微钙化,135例不伴微钙化。17 MHz高频超声对甲状腺乳头状癌微钙化的检出率为92.86%(26/28),明显高于常规超声的67.86%(19/28),差异有统计学意义(P<0.05)。17 MHz高频超声诊断甲状腺乳头状癌微钙化ROC曲线下面积(AUC)、灵敏度、特异度、符合率分别为0.930(0.904~0.978)、92.86%、90.37%、90.80%。17 MHz高频超声在诊断深度<2.6 cm的甲状腺癌微钙化符合率约为94.39%,而在诊断深度≥2.6 cm的甲状腺癌微钙化符合率为83.93%。甲状腺乳头状癌微钙化病人的甲状腺包膜受侵犯及淋巴结转移比例增高。结论17 MHz高频超声对甲状腺乳头状癌微钙化的检出率高于常规超声,甲状腺乳头状癌微钙化与不良病理特征有关。 Objective To analyze the value of 17 MHz high frequency ultrasound in the diagnosis of microcalcification in papillary thyroid carcinoma.Methods A total of 163 cases of papillary thyroid cancer patients treated in The Ninth Hospital of Xi’an from February 2016 to May 2018 were selected as the research subjects.All patients were examined by conventional ultrasound and 17 MHz high frequency ultrasound.The location,boundary,internal echo and posterior echo of thyroid nodules in patients with papil⁃lary thyroid cancer were observed and recorded.The value of 17 MHz high frequency ultrasound in the diagnosis of microcalcifica⁃tion in thyroid papillary carcinoma was analyzed by receiver operating characteristic(ROC)curve.The relationship between micro⁃calcification in papillary thyroid carcinoma and clinical pathology of papillary thyroid carcinoma was analyzed.Results Pathologi⁃cal examination confirmed that 28 of 163 cases of papillary thyroid cancer were accompanied by microcalcification and 135 cases were not accompanied by microcalcification.The detection rate of microcalcification in thyroid papillary carcinoma by 17 MHz high frequency ultrasound was 92.86%(26/28),which was significantly higher than that by conventional ultrasound(67.86%,19/28),with statistical significance(P<0.05).The area under the ROC curve(AUC),sensitivity,specificity and coincidence rate of 17 MHz high⁃frequency ultrasound in the diagnosis of microcalcification of papillary thyroid carcinoma were 0.930(0.904-0.978),92.86%,90.37%and 90.80%,respectively.The coincidence rate of 17 MHz high⁃frequency ultrasound in diagnosing microcalcifica⁃tion of thyroid cancer with depth less than 2.6 cm was about 94.39%,while that with depth greater than or equal to 2.6 cm was 83.93%.The rate of thyroid capsule invasion and lymph node metastasis in patients with thyroid papillary carcinoma microcalcifica⁃tion increased.Conclusions The detection rate of microcalcification in thyroid papillary carcinoma by 17 MHz high⁃frequency ul⁃trasound is higher than that by conventional ultrasound.Microcalcification in thyroid papillary carcinoma is related to poor pathologi⁃cal features.
