摘要
目的:探析脓毒症相关性脑病(SAE)患者血清S100β蛋白、神经元特异性烯醇化酶(NSE)的水平,以及经颅多普勒超声(TCD)血流动力学表现,为临床SAE早期诊断及预防提供指导。方法:选取2018年1月-2020年1月福建医科大学附属协和医院重症医学科收治的30例SAE患者作为研究组,将同期收治的30例无脑病脓毒症患者作为对照组,均在入院后接受血清学检测,对比分析血清S100β、NSE水平,并接受TCD检查,分析上述指标相关性。结果:研究组血清S100β、NSE均高于对照组,差异均有统计学意义(P<0.05);TCD检查中,研究组收缩期血流速度(Vs)、舒张末期血流速度(Vd)、平均流速(Vm)均低于对照组,而搏动指数(PI)、阻力指数(RI)均高于对照组,差异均有统计学意义(P<0.05);血清NSE、S100β与Vs、Vd、Vm呈负相关(P<0.05),而与PI、RI呈正相关(P<0.05)。结论:SAE患者的血清S100β、NSE及颅脑血流动力学有明显变化,具有相关性,加强血清标志物监测和TCD检查可有助于早期发现和诊断。
Objective:To investigate the serum levels of S100 protein,neuron-specific enolase(NSE)and hemodynamic manifestations of transcranial doppler ultrasound(TCD)in patients with sepsis associated encephalopathy(SAE),so as to provide guidance for early diagnosis and prevention of clinical SAE.Method:Thirty SAE patients admitted to the Department of Critical Care of Fujian Medical University Union Hospital from January 2018 to January 2020 were selected as the study group,and 30 patients with sepsis without brain disease admitted during the same period were selected as the control group,after admission,all of them received serology examination,comparative analyzed the serum S100β,NSE levels,and TCD examination,the correlation of the above indicators and the validity of early diagnosis of SAE.Result:Serum S100 and NSE in the study group were significantly higher than those in the control group,the differences were statistically significant(P<0.05).In TCD examination,systolic blood flow velocity(Vs),end-diastolic blood flow velocity(Vd)and mean flow velocity(Vm)in the study group were all lower than those in the control group,while pulse index(PI)and resistance index(RI)were higher than those in the control group,with statistically significant differences(P<0.05).Through analysis,serum NSE and S100 were negatively correlated with Vs,Vd and Vm(P<0.05),while positively correlated with PI and RI(P<0.05).Conclusion:The serum S100β,NSE and cranial hemodynamics of SAE patients change significantly,has relevance,and strengthen the monitoring of serum markers and TCD examination can help early detection and diagnosis.
