摘要
目的:研究不同类型心力衰竭患者心电图QRS波时限与血浆N末端B型利钠肽原水平的相关性。方法:本次纳入2019年1-12月收治的96例心力衰竭患者作为观察组,按照心力衰竭类型分为收缩性心力衰竭组、舒张性心力衰竭组,分别为47、49例,将同期在本院进行治疗的非心力衰竭患者100例作为对照组。比较各组心电图QRS波时限、血浆N末端B型利钠肽原水平、心功能分级、心功能超声指标,并对心电图QRS波时限与其他各项指标的相关性进行分析。结果:心电图QRS波时限、血浆N末端B型利钠肽原水平组间比较,收缩性心力衰竭组>舒张性心力衰竭组>对照组,差异有统计学意义(P<0.05);观察组中心电图QRS波时限、血浆N末端B型利钠肽原水平随NYHA心功能分级的增加而升高,差异有统计学意义(P<0.05);观察组患者的左室射血分数低于对照组,左室收缩末期内径、左室舒张末期内径高于对照组,收缩性心力衰竭组患者的左室射血分数低于舒张性心力衰竭组(P<0.05);心电图QRS波时限与左室射血分数呈负相关,与血浆N末端B型利钠肽原、NYHA心功能分级呈正相关(P<0.05)。结论:通过检测心电图QRS波时限、血浆N末端B型利钠肽原水平能够对心力衰竭的类型进行有效判断,两者之间密切相关,并能够反映心功能。
Objective:To study the correlation between QRS duration and plasma N-terminal pro-B-type natriuretic peptide in patients with different types of heart failure.Method:A total of 96 patients with heart failure admitted from January to December 2019 were selected as the observation group.According to the types of heart failure,they were divided into systolic heart failure group and diastolic heart failure group,with 47 cases and 49 cases,respectively.100 cases of non heart failure patients treated in our hospital during the same period were taken as the control group.The QRS duration of ECG,plasma N-terminal pro-B-type natriuretic peptide level,cardiac function grading,cardiac function ultrasound indexes were compared in each group,and the correlation between ECG QRS wave duration and other indicators were analyzed.Result:Compared the ECG QRS wave duration and plasma N-terminal pro-B-type natriuretic peptide level,the result showed that systolic heart failure group was higher than diastolic heart failure group,and higher than control group,the differences were statistically significant(P<0.05);the QRS wave duration of ECG and plasma N-terminal pro-B-type natriuretic peptide level in the observation group increased with the increase of NYHA cardiac function grade,and the differences were statistically significant(P<0.05).The left ventricular ejection fraction of the observation group was lower than that of the control group,the left ventricular end systolic diameter and left ventricular end diastolic diameter were higher than those of the control group,the left ventricular ejection fraction of the systolic heart failure group was lower than that of the diastolic heart failure group(P<0.05).The QRS wave duration of electrocardiogram was negatively correlated with left ventricular ejection fraction,and was positively correlated with plasma N-terminal pro-B-type natriuretic peptide and NYHA cardiac function classification(P<0.05).Conclusion:The detection of QRS duration and plasma N-terminal pro-B-type natriuretic peptide can effectively judge the type of heart failure,which are closely related and can reflect the cardiac function.
