摘要
目的探讨结直肠癌原发灶及转移淋巴结中胸腺嘧啶核苷酸合成酶(thymidylate synthase,TS)、Ki-67和p53表达差异性,并探究其对预测5-氟尿嘧啶(5-fluorouracil,5-FU)敏感性的价值。方法选取有淋巴结转移的结直肠腺癌患者33例,采用免疫组织化学方法检测结直肠癌淋巴结转移灶及原发灶中TS、Ki-67和P53的表达情况。结果TS高表达、p53和Ki-67在转移淋巴结的表达率显著高于原发灶;较好预后的即对5-FU化疗敏感的病例共7例,其中在转移淋巴结及原发癌中TS均为低表达率,p53、Ki-67为阴性表达。结论转移淋巴结中的p53、TS、Ki-67的表达情况同原发灶相比有异质性变化,转移淋巴结TS表达情况可以预测结直肠癌患者对5-FU化疗敏感性,p53的表达情况或许可预测结直肠癌患者对5-FU的化疗敏感性,Ki-67表达与5-FU化疗敏感性的关系仍需要进一步研究。
Objective To explore the diversity of the expression of thymidylate synthase(TS),Ki-67 and p53 in primary lesion and metastatic lymph nodes of colorectal cancer and its value in predicting 5-fluorouracil(5-FU)chemotherapy sensitivity.Methods The 33 cases of colorectal adenocarcinoma with metastatic lymph nodes were collected.Immunohistochemistry was used to detect the expression of TS,Ki-67 and p53 in metastatic lymph nodes and primary lesion of colorectal cancer.Results The high expression rate of TS and the positive rates of p53 and Ki-67 in metastatic lymph nodes were significant higher than those in the primary tumors.In 7 cases with good prognosis,namely sensitive to 5-FU chemotherapy,TS exppression level was low,and p53 and Ki-67 were negative in both metastatic lymph nodes and primary lesion.Conclusion The expression of TS,p53 and Ki-67 in metastatic lymph nodes has heterogeneous changes compared with the primary lesion.The expression level of TS in metastatic lymph nodes can be used to predict the sensitivity to 5-FU chemotherapy of colorectal cancer patients,the expression level of p53 possibly can be used to predict the che-motherapy sensitivity to 5-FU of colorectal cancer patients,and the relationship between the expression level of Ki-67 and sensitivity to 5-FU needs further study.
