摘要
目的评估超声引导下经会阴定位模板的前列腺投影穿刺活检(TTMB)不同阶段的手术效果,探讨既往无TTMB经验的外科医师如何尽快掌握TTMB。方法回顾性分析2017年6月至2018年6月60例行TTMB的患者的一般资料,采集两位医生各完成30例TTMB的相关数据,按照穿刺顺序,以每5例为一个学习曲线阶段,比较6个阶段手术时间、穿刺长度、穿刺失误率等穿刺效果指标,分析不同阶段的穿刺效果。结果在这6个阶段中,患者前列腺体积、年龄、体重指数(BMI)比较,差异无统计学意义(P>0.05),第3~6阶段中各项指标比较,差异无统计学意义(P>0.05)。结论既往无TTMB经验的外科医师行TTMB的学习曲线大约需要10例操作,随着操作例数增长,操作时间缩短,穿刺失误减少,穿刺效果明显改善。
Objective To evaluate the efet of transperineal template-guided mapping biopsy(TTMB)guided by ultrasound at different stages,and to explore how surgeons without previous TTMB experience master TTMB as soon as possible.Methods A retrospective analysis was made on 60 TT-MB cases completed by two doctors in the same treatment group from June 2017 to June 2018.The data of the first 30 cases of prostate projection biopsy performed by each doctor were collected.According to the puncture sequence,the puncture effect indexes such as operation time,puncture length and punc-ture error rate in six stages were compared with each other,and the puncture effect indexes such as puncture time,puncture error rate were analyzed.Puncture effect at different stages.Results There was no significant difference in prostate volume,age and BMI among the six stages.The average opera-tion time in the first stage was 68.5 minutes,puncture length was 1.20 cm,puncture error rate was 10.07%,the average operation time in the second stage was 55.4 minutes,puncture length was 1.28 cm,puncture error rate was 5.91%,the average operation time in the third stage was 46.1 minutes,puncture length was 1.34 cm,puncture error rate was 3.73%.The average operation time from the third stage to the sixth stage was 45.3 minutes,the puncture length was 1.34 cm,and the puncture er-ror rate was 3.36%.There was no significant difference between the three and six stages.Conclu-sionsThe learning curve of TTMB for surgeons without previous TTMB experience is about 10.With the increase of operation cases,the operation time is obviously shortened,the puncture errors are obviously reduced,and the puncture effect is obviously improved.
