摘要
目的探讨三维盆底超声在评估产后盆腔器官脱垂(POP)中的临床价值。方法75例POP产妇和同期80例健康产妇纳入本研究,所有受试者进行三维盆底超声检查,比较2组受试者不同状态下盆底超声参数的差异。结果静息状态下,POP组患者肛提肌裂孔前后径(5.5±0.5)cm,肛提肌裂孔横径(4.7±0.4)cm,肛提肌裂孔面积(24.4±4.2)cm2以及膀胱尿道后角(122.5±12.5)°,均明显大于对照组[(4.5±0.4)cm,(4.0±0.3)cm,(18.2±3.8)cm2,(108.4±12.9)°,P<0.05]。缩肛状态下,POP组患者肛提肌裂孔前后径(5.1±0.6)cm,肛提肌裂孔横径(4.3±0.4)cm,肛提肌裂孔面积(20.3±3.7)cm2以及膀胱尿道后角(120.5±10.8)°,均明显大于对照组[(4.0±0.5)cm,(3.7±0.4)cm,(13.2±2.5)cm2,(106.7±11.5)°,P<0.05]。Valsalva状态下,POP组患者肛提肌裂孔前后径(5.9±0.5)cm,肛提肌裂孔横径(5.0±0.4)cm,肛提肌裂孔面积(27.4±4.5)cm2,膀胱尿道后角(148.5±13.7)°,膀胱颈活动度(20.5±4.0)mm和尿道旋转角(40.2±3.8)°均明显大于对照组[(4.9±0.4)cm,(4.2±0.3)cm,(19.0±3.8)cm2,(123.4±11.8)°,(15.1±3.5)mm,(28.5±3.6)°,P<0.05]。结论POP患者盆腔器官结构改变,三维盆底超声检查有助于POP的诊断。
Objective To investigate the clinical application value of three-dimensional pelvic floor ultrasound in the evaluation of postpartum pelvic organ prolapse(POP).Methods A total of 75 parturients with POP and 80 healthy parturients were enrolled in the study.All the subjects were examined by three-dimensional ultrasound,and the differences of ultrasound parameters were compared between the two groups.Results At quiescent condition,the antero-posterior diameter of levator ani muscle slit pore,and the transverse diameter of levator ani muscle slit pore,and posterior angle of urinary bladder in POP group were significantly larger than those in control group(P<0.05).At anus constriction condition,the indexes mentinoed above in POP group were significantly larger than those in control group(P<0.05).At Valsalva condition,these indexes mentinoed above in POP group were significantly larger than those in control group(P<0.05).Conclusion The structural changes of pelvic organs exist in patients with POP.The three-dimensional ultrasound examination of pelvic floor is helpful for the diagnosis of pop.
