摘要
目的:观察暖肝煎加味对老年萎缩性胃炎合并幽门螺旋杆菌(Helicobacter Pylori,HP)感染的疗效及血清IL-6、TNF-α的影响。方法:将2017年4月~2019年11月萎缩性胃炎合并HP感染老年患者87例,随机分为2组,对照组(n=41)采用奥美拉唑、阿莫西林、克拉霉素治疗;观察组(n=46)采用暖肝煎加味,持续治疗4周。比较两组的临床疗效、中医证候积分及血清白介素-6(Interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)水平。结果:与治疗前比,两组治疗后胃脘隐疼、食少纳呆、食后饱胀、神疲懒言积分降低;与对照组比,观察组治疗后胃脘隐疼、食少纳呆、食后饱胀、神疲懒言积分较低,差异有统计学意义(P<0.05)。与治疗前比,两组治疗后血清TNF-α、IL-6水平降低;与对照组比,观察组治疗后血清TNF-α、IL-6水平较低,差异有统计学意义(P<0.05)。对照组有效率78.05%低于观察组93.48%,差异有统计学意义(P<0.05)。结论:暖肝煎加味对老年萎缩性胃炎合并HP感染的疗效显著,可降低炎症因子,改善中医证候,且安全性高,值得推广。
Objective:To investigate the effect of Modified Nuangan Decoction for senile atrophic gastritis complicated with Helicobacter pylori(HP)infection and its influence on serum IL-6 and TNF-α.Methods:87 elderly patients with atrophic gastritis complicated with HP infection from April 2017 to November 2019 were randomly divided into two groups.The controlled group(n=41)was treated with Omeprazole,Amoxicillin and Clarithromycin,the observation group(n=46)was treated with Nuangan Decoction,and 2 groups were treated for 4 weeks.The clinical efficacy,TCM syndrome score and serum levels of interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)were compared between the two groups.Results:Compared with before treatment,the scores of epigastralgia,anorexia,fullness after eating and mental fatigue and lazy speech in the two groups decreased after treatment,compared with the controlled group,the scores of epigastric hidden pain,less appetite,fullness after eating,mental fatigue and lazy speech in the observation group were lower after treatment,the difference was statistically significant(P<0.05).Compared with before treatment,the levels of serum TNF-αand IL-6 in the two groups were lower after treatment,compared with the controlled group,the levels of serum TNF-αand IL-6 in the observation group were lower after treatment,and the difference was statistically significant(P<0.05).The effective rate of the controlled group(78.05%)was lower than that of the observation group(93.48%)(P<0.05).Conclusion:Using Modified Nuangan Decoction in the treatment of senile atrophic gastritis complicated with HP infection for senile gastritis is significant,and it is worthy of promotion.
