摘要
4C延续性护理是一种新型的护理模式,以美国的奥马哈系统(omaha system)为理论框架,其内容涉及一系列具有全面性(comprehensiveness)、合作性(collaboration)、协调性(coordination)及延续性(continuity)的护理活动。该模式区别于传统的延续性护理,弥补了既往护理模式忽视患者心理状态的缺陷,注重全身心护理,已在慢性疾病、恶性肿瘤、妇产科等患者中得以应用。本文总结了4C延续性护理模式在我国临床护理中的应用现状,包括其起源及框架、应用、优势和不足等,以期该模式更广泛地应用于护理工作,促进患者康复,提高护理质量。
4C continuous care,taking American Omaha System as theoretical framework,is a new type of care model,which involves a range of care activities with comprehensiveness,collaboration,coordination and continuity.This model is different from the traditional continuous care.It makes up for the defect that the previous care model ignores the psychological state of patients,and pays attention to the whole body care.So far,it has been applied in patients with chronic diseases,malignant tumors,patients in the department of obstetrics and gynecology and so on.This article summarizes the current status of the application of the 4C continuous care model in clinical care in china,including its origin and framework,application,advantages and disadvantages,so as to make the model more widely used in nursing work,thus accelerating patient rehabilitation and improving care quality.
