摘要
目的研究不同免疫检验法对乙肝病毒感染血清标志物检验的效果。方法选取合肥市瑶海区第二人民医院2019年5月至2020年5月收治的临床确诊为乙肝的患者40例(乙肝指标处于临界值附近)进行研究,同时进行酶联免疫吸附法(ELISA)和化学发光免疫分析法(ECLIA)检测。对比两种检测方法的检出率和成本。结果ELISA方法检测乙肝表面抗原(HBsAg)30例(75.00%),乙型肝炎e抗原(HBeAg)7例(17.50%),乙肝e抗体(HBeAb)7例(17.50%);ECLIA方法检测HBsAg 37例(92.50%),HBeAg 15例(37.50%),HBeAb 16例(40.00%),两种检测方法比较差异有统计学意义(P<0.05)。两种检测方法的乙肝表面抗体(HBsAb)、乙肝核心抗体(HBcAb)检出率比较差异无统计学意义(P>0.05)。ELISA方法检测准确率33例(82.50%),成本为(20.00±2.00)元;ECLIA方法检测准确率38例(95.00%),成本为(170.00±5.00)元,两种检测方法比较差异有统计学意义(P<0.05)。结论在检测乙肝病毒血清标志物中,应用ECLIA检测方法更能达到理想的阳性检测率,相比于ELISA检测,HBsAb和HBcAb阳性检出率并无差异,而在HBsAg、HBeAg、HBeAb阳性检出率上,ECLIA检测检出率更高,但检测成本远高于ELISA检测,因此,应当根据临床实际情况进行选择。
Objective To study the effect of different immunoassays on serum markers of hepatitis B virus infection.Methods From May 2019 to May 2020,40 patients who were clinically diagnosed with hepatitis B in the Second People's Hospital of Yaohai District were selected for the study.Enzyme-linked immunosorbent assay(ELISA)and chemiluminescent immunoassay(ECLIA)were used simultaneously.Compare the detection rate and cost of the two methods.Results ELISA was used to detect HBsAg in 30 cases(75.00%),HBeAg in 7 cases(17.50%)and HBeAb in 7 cases(17.50%).ECLIA was used to detect HBsAg in 37 cases(92.50%),HBeAg in 15 cases(37.50%)and HBeAb in 16 cases(40.00%).There was a significant difference between the two methods(P<0.05).There was no significant difference in the detection rates of HBsAb and HBcAb between the two methods(P>0.05).The detection accuracy of ELISA method was 33 cases(82.50%),and the cost was(20.00±2.00)yuan;the detection accuracy of ECLIA method was 38 cases(95.00%),and the cost was(170.00±5.00)yuan.There was a significant difference between the two detection methods(P<0.05).Conclusion Compared with ELISA,there is no difference in the positive detection rates of HBsAb and HBcAb.In terms of the positive detection rates of HBsAg,HBeAg and HBeAb,the detection rate of ECLIA is higher,but the detection cost is much higher than that of ELISA,so it should be selected according to the actual situation.
