摘要
目的:探讨苏子降气汤加减联合西药治疗老年慢性阻塞性肺疾病(COPD)急性加重期患者疗效及对血清D-二聚体(D-D)、超敏C反应蛋白(hs-CRP)的影响。方法:120例老年COPD急性加重期患者,将编号为奇数者纳入对照组(n=60),编号为偶数者纳入研究组(n=60)。两组都给予常规治疗,对照组在常规治疗基础上采用多索茶碱静滴,研究组在对照组基础上联合苏子降气汤加减治疗。比较两组疗效,患者血清D-D、hs-CRP的水平。结果:研究组总有效率93.33%,对照组总有效率76.67%,两组比较差异有统计学意义(P<0.05)。治疗后研究组中医症状积分低于对照组(P<0.05)。研究组治疗后血沉、降钙素原、血清D-D、纤维蛋白原水平均低于对照组(P<0.05)。治疗后两组第1秒用力呼气容积、用力肺活量均升高,FEV1/FVC水平降低,且研究组治疗后FEV1、FVC高于对照组,FEV1/FVC低于对照组,差异有统计学意义(P<0.05)。结论:苏子降气汤加减联合西药治疗能改善老年COPD急性加重期患者中医证候、凝血功能及肺功能,减轻炎症反应,疗效较好。
Objective:To investigate the effect of modified Suzi Jiangqi decoction combined with western medicine on serum D-Diner(D-D)and high sensitivity C-reactive protein(hs-CRP)in elderly patients with acute exacerbation of chronic obstructive pulmonary disease(COPD).Methods:120 elderly patients with acute exacerbation of COPD were selected.Those with odd numbers were included in the control group(n=60)and those with even numbers in the study group(n=60).Both groups were given conventional treatment.On this basis,the control group was treated with intravenous drip of doxofylline,while the study group was treated with modified Suzi Jiangqi decoction combined with intravenous drip of doxofylline.Clinical effects were comparatively analyzed,and effects of different treatment methods on serum D-D and hs-CRP were analyzed.Results:The total effective rate was 93.33%in the study group and 76.67%in the control group,significant difference(P<0.05).After treatment,TCM symptom scores of the study group were significantly lower than those of the control group(P<0.05).The levels of hs-CRP,erythrocyte sedimentation rate,procalcitonin,serum D-D and fibrinogen in the study group were significantly lower than those in the control group(P<0.05).After treatment,the forced expiratory volume in 1 second(FEV1)and forced vital capacity of the two groups significantly increased,while FEV1/FVC decreased.Besides,FEV1 and FVC in the study group were significantly higher than those in the control group,and FEV1/FVC was lower than that in the control group,significant difference(P<0.05).Conclusion:Modified Suzi Jiangqi decoction combined with western medicine can significantly improve TCM symptoms,blood coagulation function and lung function in elderly patients with acute exacerbation of COPD while reliving inflammation,with better curative effects.
