摘要
慢性萎缩性胃炎是临床上常见的一种胃癌前病变,情志失调是其发生发展过程中的重要危险因素。“调枢通胃”理论植根于脾胃学说,创新地提出了通调脏腑之枢脾胃、神明之枢心脑以及开阖之枢少阳来治疗疾病,三条枢机与情志的致病特点密切相关,对临床诊治慢性萎缩性胃炎(chronic atrophic gastritis,CAG)伴情志不遂这类情志病有指导作用。神明之枢心脑为情志病的发病之所,脏腑之枢脾胃为情志失调最常见的受病之所,两者离不开经络的联通作用。据此,从调肝理脾——通调脏腑之枢、开窍安神——通调神明之枢、和解少阳——通调开阖之枢来针药并用,治疗与调护并重,对患者进行个性化情志疏导,可获良效,在临床有广泛的应用前景。
Chronic atrophic gastritis(CAG)is a common precancerous lesion of the stomach in clinical practice.Emotional factors are important risk factors in their occurrence and development.Those theory of‘regulating pivot to smoothing stomach qi’is rooted in the theory of the spleen and stomach.It has been innovatively proposed that regulating the spleen and stomach of the pivot of zang-fu viscera,regulating the heart and brain of the pivot of spirit,regulating shaoyang of the pivot of open-closed,in order to treat disease.Those three pivots are closely related to the pathogenic characteristics of emotions.There is guiding role to clinical diagnosis and treatment of CAG associated with emotional dysfunction.The heart and brain of the pivot of spirit,is the occurrence place of emotion disease.The spleen and stomach of the pivot of zang-fu viscera,is the most common bearing disease place of emotion disorder.They are inseparable from the connecting function of channels and collaterals.Accordingly,from the aspects of regulating the liver and spleen,namely regulating the pivot of the zang-fu viscera inducing resuscitation and tranquillization,namely regulating the heart and brain of the pivot of spirit,and harmonizing Shaoyang,namely regulating the pivot of open-closed,to combine acupuncture with medication,putting treatment and care on equal important,conducting personalized emotional counseling for patients.It can obtain good effect and has a wide application prospect in clinical practice.
