摘要
目的:探讨大黄穴位贴敷联合奥美拉唑钠预防骨折患者应激性黏膜病变(SRMD)的临床效果。方法:选取2018年6月-2019年6月本院骨科患者100例,将其随机分为对照组和观察组,各50例。两组均予基础治疗静脉注射奥美拉唑钠,对照组予以基础护理措施,观察组在对照组基础上予以大黄穴位贴敷治疗。观察两组干预前后粪便隐血试验阳性率、血清胃泌素(gastrin,GAS)水平和SRMD发生情况。结果:干预后,观察组的隐血试验阳性率4.00%,明显低于对照组的22.00%,差异有统计学意义(P<0.05)。干预后,两组GAS水平均较干预前明显降低,且观察组GAS水平明显低于对照组,差异均有统计学意义(P<0.05)。干预后,观察组SRMD发生率2.00%,明显低于对照组20.00%,差异有统计学意义(P<0.05)。结论:大黄穴位贴敷联合奥美拉唑钠,通过西医治疗及中医特色技术结合预防方案,迅速改变胃内酸性环境,促进胃肠蠕动恢复,保护胃黏膜屏障,为骨折SRMD患者探寻一种安全、有效、副作用少的中医特色护理方案。
Objective:To explore the clinical effect of Rhubarb acupoint application combined with Omeprazole Sodium in the prevention of stress related mucosal disease (SRMD) in patients with fractures.Method:Atotal of 100 orthopaedic patients in our hospital from June 2018 to June 2019 were selected.They were randomly divided into control group and observation group,50 cases in each group.Both groups were injected with Omeprazole Sodium for basic treatment,the control group was given basic nursing measures,and the observation group was given Rhubarb acupoint application treatment on the basis of the control group.The positive rate of faecal octant test,serum gastrin (GAS) level and the occurrence of stress related mucosal disease were observed before and after intervention in the two groups.Result:After intervention,the positive rate of occult blood test in the observation group was 4.00%,which was significantly lower than 22.00% in the control group,with statistically significant difference (P<0.05).After intervention,GAS levels in both groups were significantly lower than those before intervention,and GAS levels in the observation group was significantly lower than that in the control group,with statistically significant differences (P<0.05).After intervention,the incidence of stress related mucosal disease in the observation group was 2.00%,significantly lower than 20.00% in the control group,the difference was statistically significant (P<0.05).Conclusion:Rhubarb acupoint application combined with Omeprazole Sodium,through western medicine treatment and traditional Chinese medicine characteristic technology combined with prevention program,can rapidly change the acidic environment of the stomach,promote the recovery of gastrointestinal motility,protect the gastric mucosal barrier,and explore a safe TCM characteristic nursing program for SRMD patients with fractures,with fewer side effects.
