摘要
目的:探讨冠心病患者血浆可溶性信号素4D(Soluble semaphorin 4D,sSema4D)水平与冠状动脉狭窄程度相关性。方法:选取2017年1月至2019年1月112例在我院接受冠状动脉血管造影检查,且狭窄<50%的患者作为对照组(n=32);同时将确诊冠心病的患者作为冠心病组(n=80),所有冠心病组患者依据Gensini积分再次分为低分组(Gensini积分<30分)和高分组(Gensini积分≧30分),采用罗氏Cobas 8000全自动生化分析仪测定血脂等生化指标,胶乳增强免疫比浊法测定hs-CRP水平,免疫荧光法测定NTproBNP水平,酶联免疫吸附法检测患者血浆sSema4D水平;同时采用Pearson进行相关性分析,Logistics回归模型进行危险因素分析。结果:冠心病组sSema4D水平、Gensini评分明显高于对照组(P<0.05),且sSema4D水平与Gensini评分正相关(r=0.476,P<0.05)。Gensini高分组患者年龄、男性比例、低密度脂蛋白(Low density lipoprotein,LDL)、超敏C反应蛋白(Hypersensitive C-reactive protein,Hs-CRP)、N端前脑钠肽(N-terminal probrain natriuretic peptide,NTproBNP)、sSema4D水平明显高于低分组(P<0.05)。Logistics回归分析显示,年龄、男性、LDL、Hs-CRP、NTproBNP、sSema4D均为高Gensini积分的独立危险因素。ROC曲线表明,sSema4D曲线下面积为0.712(95%CI:0.590-0.796)。当sSema4D诊断临界值取10126 pg•ml-1时,敏感性为82.6%,特异性为87.5%。结论:sSema4D水平与Gensini评分正相关,对冠心病的严重程度具有较好的预测价值。
Objective:To investigate the correlation between plasma soluble Sema4D(sSema4D)level and coronary artery stenosis in patients with coronary heart disease.Methods:From January 2017 to January 2019,112 patients underwent coronary angiography.The patients with stenosis<50%were used as a control group(n=32),and patients with coronary heart disease were diagnosed as coronary heart disease(n=80),in which patients with coronary heart disease were divided into low-group(Gensini score<30 points)and high-group(Gensini score≧30 points).Roche Cobas 8000 automatic biochemical analyzer was used to measure blood lipids and other biochemical indicators.Latex-enhanced immunoturbidimetric method was used to detect measure hs-CRP level.The immunofluorescence method was used to detect measure NTproBNP level.Enzyme-linked immunosorbent assay was used to detect the plasma soluble semaphorin 4D(sSema4D)level.Pearson was used for correlation analysis.Logistics regression model was used for risk factors.Results:The sSema4D level and Gensini score in the CHD group were significantly higher than those in the control group(P<0.05).The sSema4D level was positively correlated with the Gensini score(r=0.476,P<0.05).Gensini high group age,male ratio,low density lipoprotein(LDL),high-sensitivity C-reactive protein(Hs-CRP),N-terminal pro-brain natriuretic peptide(NTproBNP),sSema4D levels were significantly higher than the control group(P<0.05).Logistic regression analysis showed that age,male,LDL,Hs-CRP,NTproBNP,and sSema4D were independent risk factors for high Gensini scores.The ROC curve indicates that the area under the curve of Sema4D was 0.712(95%CI:0.590-0.796).When the diagnostic value of Sema4D was 10126 pg•ml-1,the sensitivity was 82.6%and the specificity was 87.5%.Conclusion:The level of sSema4D is positively correlated with the Gensini score,and has a good predictive value for the severity of coronary heart disease.
