摘要
目的探讨表皮葡萄球菌ica A/ica D基因和前列腺液中性粒细胞弹性蛋白酶(NE)联合检测的临床意义。方法选取2013年1月至2015年12月在黄河三门峡医院就诊的表皮葡萄球菌性前列腺炎患者657例,收集患者分离出的657株表皮葡萄球菌检验结果以及临床资料。采用聚合酶链反应方法检测表皮葡萄球菌ica A/ica D基因水平,酶联免疫吸附试验法检测前列腺液NE含量。结果657株表皮葡萄球菌的ica A阳性率为19.6%(129株),ica D阳性率为21.3%(140株)。657例患者中,273例(41.6%)前列腺液中NE含量≤800μg/L;63例(9.6%)前列腺液中NE含量>800且<1000μg/L;321例(48.9%)前列腺液中NE含量≥1000μg/L。ica A阳性的表皮葡萄球菌感染者中NE≥1000μg/L者比例为72.1%(93/129),ica D阳性的表皮葡萄球菌感染者中NE≥1000μg/L的比例为72.9%(102/140)。结论表皮葡萄球菌ica A/ica D基因和前列腺液中NE联合检测对表皮葡萄球菌性前列腺炎诊疗方案的制定有一定的指导意义。
Objective To investigate the clinical significance of combined detection of Staphylococcus epidermidis ica A/ica D gene and neutrophil elastase(NE)level in prostatic fluid.Methods The clinical data of657 patients with Staphylococcus epidermidis prostatitis in Sanmenxia Hospital of the Yellow River from January2013 to December 2015 and detected results of 657 strains Staphylococcus epidermidis from the patients were collected.The ica A/ica D gene level of Staphylococcus epidermidis was detected by polymerase chain reaction,and the content of NE in prostatic fluid was detected by enzyme-linked immunosorbent assay.Results The positive rate of ica A in 657 strains of Staphylococcus epidermidis was 19.6%(129/657),and the positive rate of ica D was 21.3%(140/657).In 273(41.6%)patients,the content of NE in prostatic fluid was≤800μg/L;in63 patients(9.6%),the content of NE in prostatic fluid was>800 and<1000μg/L;in 321 cases(48.9%),the NE content in prostatic fluid was≥1000μg/L.The proportion of NE≥1000μg/L was 72.1%(93/129)in patients with ica A positive Staphylococcus epidermidis infection,and 72.9%(102/140)in patients with ica D positive Staphylococcus epidermidis infection.Conclusion The combined detection of ica A/ica D gene of Staphylococcus epidermidis and NE level in prostatic fluid has certain guiding significance for the development of clinical diagnosis and treatment of prostatitis.
