摘要
目的探讨经皮椎间孔脊柱内窥镜术联合腰痛宁治疗腰椎间盘突患者的效果。方法选取2019年2月至2020年2月于本院行手术治疗的腰椎间盘突出患者106例,将其分为对照组57例和试验组49例。对照组实施常规开放手术联合常规药物治疗,试验组实施经皮椎间孔脊柱内窥镜术联合腰痛宁治疗,对比两组腰椎功能和各炎性因子水平。结果试验组主观症状、日常活动受限、临床体征等评分及总分显著优于对照组[(7.67±1.75)分比(5.25±1.34)分、(12.35±2.56)分比(8.75±1.67)分、(6.97±0.94)分比(4.25±0.64)分、(22.78±3.54)分比(18.57±2.65)分,均P<0.05];试验组5羟色胺(5-HT)、前列腺素E2(PGE2)、白细胞介素-1β(IL-1β)及肿瘤坏死因子-α(TNF-α)水平显著低于对照组[(135.24±25.37)pg·ml^-1比(167.23±28.46)pg·ml^-1、(215.31±35.06)pg·ml^-1比(246.25±38.12)pg·ml^-1、(26.34±6.25)pg·ml^-1比(34.16±8.52)pg·ml^-1、(17.45±5.12)pg·ml^-1比(24.16±6.34)pg·ml^-1,均P<0.05]。结论经皮椎间孔脊柱内窥镜术联合腰痛宁应治疗腰椎间盘突出患者可显著改善患者临床症状和体征,促进其机体功能恢复,减少机体细胞炎症的发生,减少并发症发生。
Objective To investigate the effect of percutaneous transpedicular endoscopic discectomy combined with Yaotongning for patients with lumbar disc herniation.Methods From February,2019 to February,2020,106 patients with lumbar disc herniation treated at our hospital were divided into a control group(n=57)and an experimental group(n=49).The control group was treated with conventional open surgery and drug,and the experimental group with percutaneous transpedicular endoscopic discectomy and Yaotongning.The spinal function and inflammation factors were compared between these two groups.Results The scores of subjective symptoms,limited daily activity,and clinical signs and the total score were significantly better in the observation group than in control group[(7.67±1.75)vs.(5.25±1.34),(12.35±2.56)vs.(8.75±1.67),(6.97±0.94)vs.(4.25±0.64),and(22.78±3.54)vs.(18.57±2.65);all P<0.05].The serum levels of serotonin(5-HT),prostaglandin E2(PGE2),interleukin-1β(IL-1β),and tumor necrosis factor-α(TNF-α)were significantly lower in the observation group than in the control group[(135.24±25.37)pg·ml^-1 vs.(167.23±28.46)pg·ml^-1,(215.31±35.06)pg·ml^-1 vs.(246.25±38.12)pg·ml^-1,(26.34±6.25)pg·ml^-1 vs.(34.16±8.52)pg·ml^-1,and(17.45±5.12)pg·ml^-1 vs.(24.16±6.34)pg·ml^-1;all P<0.05].Conclusion Percutaneous transpedicular endoscopic discectomy combined with Yaotongning for patients with lumbar disc herniation can significantly improve their clinical symptoms and signs,promote their recovery of body function,and reduce the occurrence of cell inflammation and the incidence of complications.
