摘要
目的:观察保乳术联合前哨淋巴结活检治疗早期乳腺癌的临床效果。方法:抽取2014年1月至2017年5月南阳市中心医院收治的早期乳腺癌患者180例,以随机数字表法分为参照组与观察组,每组90例。参照组行乳房全切联合腋窝淋巴结清扫术治疗,观察组行保乳术联合前哨淋巴结活检治疗。比较两组手术时间、术中出血量、引流管拔除时间、住院时间,术后1、3年内复发率、转移率及并发症发生率。结果:观察组手术时间、引流管拔除时间、住院时间短于参照组,术中出血量低于参照组,差异有统计学意义(P<0.05);观察组1年内复发率、转移率与参照组比较,差异未见统计学意义(P>0.05)。观察组3年内复发率、转移率与参照组比较,差异未见统计学意义(P>0.05);观察组并发症发生率低于参照组,差异有统计学意义(P<0.05)。结论:早期乳腺癌患者选择保乳联合前哨淋巴结活检术治疗,能够缩短患者手术时间、住院时间,降低术后并发症发生率,且复发率、转移率较低。
Objective To observe the clinical effect of breast-conserving surgery combined with sentinel lymph node biopsy in the treatment of early breast cancer.Methods A total of 180 patients with early breast cancer who were admitted to Nanyang Central Hospital from January 2014 to May 2017 were randomly divided into the control group and the observation groups,with 90 cases in each group.The control group received total mastectomy combined with axillary lymph node dissection,the observation group was treated with breast-conserving surgery combined with sentinel lymph node biopsy.The operation time,intraoperative blood loss,extubation time,hospitalization time,recurrence rate and metastasis rate,the incidence of complications and quality of life of patients were compared between the two groups.Results The operation time,drainage tube extraction time,hospitalization time in observation group were shorter than those in the control group,the intraoperative bleeding volume was lower in observation group than that in the control group,the difference was statistically significant(P<0.05),There was no significant difference in recurrence rate and metastasis rate between observation group and control group within 1 year,there was no significant difference(P<0.05).There was no significant difference in recurrence rate and metastasis rate between observation group and control group within 3 years(P<0.05),and the complication rate in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusions Breast conserving surgery combined with sentinel lymph node biopsy can shorten the time of operation and hospitalization,reduce the incidence of postoperative complications for patients with early breast cancer,and the recurrence rate,the transfer rate is low.
