摘要
目的探讨睡眠质量与糖尿病缺血性脑卒中发病风险的相关性,为糖尿病脑卒中的发生提供一定的预测价值。方法从2011年4月~2011年11月参与“四川泸州地区2型糖尿病患者肿瘤发生风险的流行病学调查”的受试者中,筛选出符合本研究标准的2型糖尿病患者358例,根据睡眠质量分为睡眠质量好组(n=271)、睡眠质量差组(n=64)与安眠药助眠组(n=23),并于2016年6~2016年11月开展随访。比较3组受试者的脑卒中发生情况,采用Logistic回归分析探讨糖尿病脑卒中发生的相关因素。结果5年随访后,3组受试者脑卒中的发生比例比较,差异有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,使用安眠药助眠糖尿病患者发生脑卒中风险是睡眠质量好患者的4.202倍,睡眠质量差与睡眠质量好糖尿病患者发生脑卒中的风险无统计学差异(P>0.05)。结论使用安眠药助眠是糖尿病发生缺血性脑卒中的危险因素,应密切关注糖尿病患者睡眠质量,并对其进行干预与指导,以便有效控制糖尿病缺血性脑卒中的发生。
Objective To explore the correlation between sleep quality and the risk of diabetic stroke,and to provide a predictive value for the occurrence of diabetic stroke.Methods From April 2011 to November 2011,participants in the"epidemiological survey of cancer risk in patients with type 2 diabetes in Luzhou,Sichuan Province"were enrolled.358 patients with type 2 diabetes mellitus were selected and divided into good sleep quality group(n=271),poor sleep quality group(n=64)and hypnotic aid group(n=23),according to sleep quality.The incidence of stroke was compared among the three groups.Logistic regression analysis was used to explore the related factors of diabetic stroke.Results After 5-year follow-up,there was a significant difference in the incidence of stroke among the three groups(P<0.05).The results of multivariate logistic regression analysis showed that the risk of stroke in diabetic patients with sleep aid was 4.202 times higher than that in patients with good sleep quality.There was no significant difference in the risk of stroke between patients with poor sleep quality and those with good sleep quality(P>0.05).Conclusion The use of hypnotics is a risk factor of ischemic stroke in diabetes mellitus.We should pay close attention to the sleep quality of diabetic patients,and provide effective intervention and guidance,so as to effectively control the occurrence of diabetic ischemic stroke.
