摘要
目的探讨医疗失效模式与效应分析在优化成人非计划性拔管管理流程中的应用效果。方法采用前瞻性随机对照试验的方法,将2019年1~5月收治入院的1215例成人患者与6~10月收治入院的1319例患者分为对照组与试验组,分别进行非计划性拔管事件常规护理管理及HFMEA模式优化的管理,分析两组非计划性拔管发生情况、患者满意度及试验组干预前后的风险系数差异。结果试验组UEX事件19例明显低于对照组43例(P <0.05)。试验组患者满意度(47.67±1.52)分,高于对照组的(42.67±2.08)分(P <0.05);试验组在项目实施前后的风险优先指数(RPN评分)为(79.67±3.51)分、(46.33±3.06)分,实施后有明显降低(P <0.05)。结论通过医疗失效模式与效应分析(HFMEA)优化成人非计划性拔管管理流程,明显降低了各环节的风险优先指数,减少了UEX事件的发生,有助于患者满意度提高。
Objective To explore the application effect of health failure mode and effects analysis(HFMEA)in the optimization of the management process of unplanned extubation(UEX)in adults.Methods A prospective,randomized,controlled trial was performed,where 1215 patients admitted to the hospital from January 2019 to May 2019 and 1319 patients admitted to the hospital from June 2019 to October 2019 were included and divided into a test group and a control group.Routine nursing management of UEX events and the optimized management of UEX events were performed for the test group and the control group,respectively.The incidence of UEX and patient satisfaction in both groups were analyzed,and the risk priority number(RPN)before and after intervention in the test group were compared.Results The incidence of UEX events in the test group was significantly lower than that of the control group(19 vs 43,P<0.05).The test group got a higher score of patient satisfaction than the control group(47.67±1.52 vs 42.67±2.08,<P<0.05).The RPN scores of the test group before and after the intervention were 79.67±3.51 and 46.33±3.06,respectively,showing a marked decline(P<0.05).Conclusion Application of HFMEA in the optimization of the management process of UEX in adults can remarkably reduce the RPN,decrease the incidence of UEX events,and improve patient satisfaction.
