摘要
目的结合《医宗金鉴》中的思想启发当下糖尿病足溃疡(diabetic foot,DF)的诊疗思路,以古为今用。方法通过分析相关原文以及现代研究成果,提出《医宗金鉴》对于DF的认识,阐述其治疗原则以及相关理法方药的理论依据,根据相关症状来判断DF预后情况。结果DF病因病机总体为热盛阴伤,后期气血毒热搏结,下发为病。治疗上有内治及外治法,外治常用外科手术、艾灸、敷贴及膏药、中药足浴等,均有满意疗效;内治法大致分为清热解毒法、养阴清热法、清热安神法、益气养血法,不仅在医宗金鉴中记载相关方剂,其思想在当代从内治疗DF也有所体现。结论探讨《医宗金鉴》中DF的辨治,对于临床指导治疗DF具有现实意义。
Objective To provide theoretical references for improving efficacy in treating diabetic foot,by analyzing the related contents in the Golden Mirror of Medicine.Method By analyzing the related contents and the results of recent researches,to conclude the treatments of DF based on the Golden Mirror of Medicine,and to elaborate its principles on treatment,the basic rules of prescriptions,and the prognosis of DF by assessing the symptoms.Results The pathogenesis of DF in early stages was generally exuberance of heat toxin with ying deficiency.In late stages,it was qi and blood stagnation with the exuberance of heat toxin.On the treatment,both internal and external therapy,which includes surgery,moxibustion,sticking therapy and foot bath therapy,indicates good efficacy.The principles of internal therapy can be roughly divided into clearing heat toxin,nourishing yin,calming the mind,and replenishing qi and nourishing blood.Those principles are not only found in the Golden Mirror of Medicine,but also considered as crucial principles for contemporary practitioners in the treatment of DF.Conclusion By discussing the differentiation of treatments of DF based on the Golden Mirror of Medicine,the principles and treatments of DF we conclude are practical and beneficial to clinical practice.
