摘要
[目的]整理总结黄挺教授治疗原发性肝癌相关抑郁状态的特色经验。[方法]通过门诊跟师学习,面授教学,收集并整理相关医案,试从病因病机、基本治法、基本用方等方面分析总结黄挺教授治疗原发性肝癌相关抑郁状态的经验,并举验案一则予以佐证。[结果]黄挺教授认为原发性肝癌相关抑郁状态之病机基本可概括为"肺脾气虚为本""癌毒气郁为标",脾虚肝郁是病机核心,治疗上以平补肺脾、柔木解郁、解毒抑癌为基本治法,总结出自拟五花汤,随证加减,屡获良效。所举验案乃肝郁脾虚之证,治以平补肺脾、柔木解郁、解毒抑癌,以自拟五花汤为底方,临证加减,标本兼治,疗效颇佳。[结论]黄挺教授以平补肺脾为根本大法,辅以柔木解郁、解毒抑癌,治疗原发性肝癌相关抑郁状态,经验独到,疗效显著,值得学习借鉴。
[Objective] To summarize Professor HUANG Ting’s characteristic experience in treating depression associated with primary liver cancer.[Methods] Through studying with the teacher through outpatient clinic, face-to-face teaching, collecting and organizing related medical records,Professor HUANG Ting’s experience in treating depression associated with primary liver cancer was summarized and analyzed from etiology and pathogenesis, basic treatment, basic prescription, and one case was presented as evidence. [Results] Professor HUANG Ting believes that the pathogenesis of primary liver cancer-related depression can be basically summarized as "pulmonary and spleen Qi deficiency as the root cause " and"cancer toxin and Qi stagnation as the manifestation ". Spleen deficiency and liver depression is the core of the pathogenesis. Tonifying the lung and spleen, softening wood to relieve depression, detoxifying and suppressing cancer are the basic treatments. The self-made Wuhua Decoction is summed up and revised with the syndromes, and it has obtained good results repeatedly. The test case cited was the syndrome of liver depression and spleen deficiency. The treatment was to tonify the lung and spleen, soften the wood to relieve depression, detoxify and suppress cancer. The self-made Wuhua Decoction was used and the patient achieved good results. [Conclusion] Professor HUANG Ting has unique experience and remarkable curative effect in the treatment of depression associated with primary liver cancer with the basic method of tonifying the lung and spleen, supplemented with softening wood to relieve depression,detoxification and tumor suppression, which is worth learning from.
