摘要
目的:探讨应用锁骨上侧入路改良小切口甲状腺切除术治疗甲状腺结节的效果及并发症发生情况。方法:选取笔者所在医院2019年2月-2020年2月确诊并治疗的86例甲状腺结节患者,依据手术方式的不同将其分为对照组和研究组,每组34例。对照组应用传统手术,研究组应用锁骨上侧入路改良小切口手术。比较两组术中相关指标、康复相关指标及并发症发生情况。结果:两组手术时长、术中出血量比较差异均无统计学意义(P>0.05);研究组切口长度小于对照组,差异有统计学意义(P<0.05)。两组平均住院时长比较差异无统计学意义(P>0.05)。研究组外观满意评分高于对照组,差异有统计学意义(P<0.05)。对照组发生2例(4.7%)喉返神经损伤,研究组发生1例(2.3%)喉返神经损伤。两组术后并发症发生率比较差异无统计学意义(P>0.05)。结论:应用锁骨上侧入路改良小切口甲状腺切除术治疗甲状腺结节,手术切口更小,患者对外观的满意度更高,安全性好。
Objective:To investigate the effect and complications of modified small incision thyroidectomy with supraclavicular approach in the treatment of thyroid nodules.Method:A total of 86 patients with thyroid nodules diagnosed and treated in our hospital from February 2019 to February 2020 were selected and divided into the control group and the study group according to different surgical methods,with 34 cases in each group.The control group was treated with traditional surgery,and the study group was treated with modified small incision with supraclavicular approach surgery.The intraoperative related indexes,rehabilitation related indexes and complications were compared between the two groups.Result:There were no significant differences in the surgery time and intraoperative blood loss between the two groups(P>0.05).The incision length of the study group was smaller than that of the control group,the difference was statistically significant(P<0.05).There was no significant difference in the average length of hospital stay between the two groups(P>0.05).The appearance satisfaction score of the study group was higher than that of the control group,the difference was statistically significant(P<0.05).There were 2 cases(4.7%)of recurrent laryngeal nerve injury in the control group,and 1 case(2.3%)in the study group.There was no significant difference in postoperative complications between the two groups(P>0.05).Conclusion:The application of modified small incision thyroidectomy with supraclavicular approach in the treatment of thyroid nodules has smaller surgical incision,higher satisfaction of patients with appearance and good safety.
