摘要
目的分析糖尿病肾病(DN)患者血清miR-155和miR-146a的表达水平,并探讨二者对糖尿病肾病的诊断价值。方法选取2016年8月至2018年8月延安大学附属医院收治的150例糖尿病患者为研究对象,根据糖尿病肾病诊断标准将糖尿病患者分为T2DM组(78例)和DN组(72例),另选取同期老年病科56例无糖尿病肾病患者作为对照组,分别检测及收集所有受试者的临床资料与生化指标,采用实时荧光定量PCR(qRT-PCR)法检测血清miR-155和miR-146a表达水平,比较miR-155和miR-146a在DN早期诊断中的价值。结果 T2DM组、DN组患者空腹血糖(FPG)、HbA1c水平明显高于对照组(P<0.05),GFR明显低于对照组(P<0.05);DN组患者FPG、HbA1c水平明显高于T2DM组(P<0.05),GFR明显低于T2DM组(P<0.05);DN组患者血清中miR-155和miR-146a水平显著低于对照组、T2DM组(P<0.05),T2DM组患者血清miR-155和miR-146a水平显著低于对照组(P<0.05);血清miR-155、miR-146a水平单独及二者联合诊断DN患者的曲线下面积分别为0.842、0.778、0.910;二者联合诊断DN价值更优。结论 miR-155、miR-146a诊断早期DN价值较高,为评估糖尿病患者肾损伤的可靠指标。
Objective To detect the levels of serum miR-155 and miR-146a in patients with diabetic nephropathy(DN),and to explore their value in the early diagnosis of DN.Methods 150 cases of diabetic patients admitted to Yan an University Affiliated Hospital from August 2016 to August 2018 were selected as the research objects.According to the diagnostic criteria of diabetic nephropathy,diabetic patients were divided into type 2 diabetes mellitus(T2DM)group(78 cases)and DN group(72 cases),and 56 healthy persons were selected as control group.The clinical data and biochemical indexes of all subjects were detected and collected,and real-time fluorescence quantitative polymerase chain reaction(qRT-PCR)was used detect the serum levels of miR-155 and miR-146a.The value of miR-155 and miR-146a in the early diagnosis of DN was compared.Results The levels of fasting plasma glucose(FPG)and glycated hemoglobin(HbAlc)in the T2DM group and the DN group were significantly higher than those in the control group(P<0.05),and the glomerular filtration rate(GFR)was significantly lower than the control group(P<0.05);the levels of FPG and HbAlc in the DN group were significantly higher than those in T2DM group(P<0.05),while GFR was significantly lower than the T2DM group(P<0.05);The serum levels of miR-155 and miR-146a in DN group were significantly lower than those in control group and T2DM group(P<0.05),and the serum levels of miR-155 and miR-146a in T2DM group were significantly lower than those in control group(P<0.05);the area under curve(AUC)of serum miR-155,miR-146a alone and in combination were 0.842,0.778 and 0.910 respectively;the combined diagnosis of DN had better value.Conclusions MiR-155 and miR-146a have high value in the diagnosis of early DN,which can be used as reliable indicators to evaluate renal injury.
