摘要
目的探讨阶段性康复在风湿性心脏瓣膜病(RVHD)手术患者中的影响。方法将2018年3月至2019年3月在某院行常规护理的35例RVHD手术患者纳入对照组;将2019年4月至2020年4月在某院行阶段性康复的36例RVHD手术患者纳入观察组。比较两组围术期间相关指标(卧床、住院天数及抗凝药物用药依从性)、日常生活能力及护理满意度。结果与对照组卧床天数(9.52±2.46)d、住院天数(21.35±3.16)d、抗凝药物用药依从性(7.05±1.14)分相比较,观察组卧床天数(7.14±2.18)d、住院天数(16.93±2.87)d均较短,抗凝药物用药依从性评分(10.19±1.20)分较高,差异有高度统计学意义(t=4.318、6.173、11.298,P<0.001);两组护理后日常生活活动情况(ADL)评分高于护理前,观察组(86.11±11.20)分高于对照组(72.38±12.09)分,差异有高度统计学意义(t=4.966,P<0.001);观察组护理总满意度优于对照组,差异有统计学意义(Z=2.089,χ2=4.772,P<0.05)。结论阶段性康复在RVHD手术患者中利于缩短术后卧床及住院时间,提高抗凝药物用药依从性、日常生活能力及护理满意度。
Objective To investigate the effect of staged rehabilitation in patients with rheumatic valvular heart disease(RVHD)surgery.Methods 35 patients undergoing RVHD surgery in a hospital from March 2018 to March 2019 treated with routine nursing were included in the control group.36 patients undergoing RVHD surgery in a hospital from April 2019 to April 2020 treated with staged rehabilitation were included in the observation group.The perioperative indicators(bed rest,length of stay in hospital and compliance with anticoagulant medication),daily life ability and nursing satisfaction were compared between the two groups.Results Compared with the control group of[bed days(9.52±2.46)d,hospital days(21.35±3.16)d,and anticoagulant medication compliance score(7.05±1.14)],the observation group had shorter bed days(7.14±2.18)d and hospital days(16.93±2.87)d,and higher anticoagulant medication compliance score(10.19±1.20),and the difference was highly statistically significant(t=4.318,6.173,11.298,P<0.001).The ADL score of the two groups after nursing was higher than that before nursing,and the score of the observation group(86.11±11.20)was higher than that of the control group(72.38±12.09),the difference was highly statistically significant(t=4.966,P<0.001).The total nursing satisfaction in the observation group was better than that in the control group,with statistically significant difference(Z=2.089,χ2=4.772,P<0.05).Conclusion In patients undergoing RVHD surgery,staged rehabilitation is beneficial to shorten the postoperative bed and hospital stay,improve the compliance of anticoagulant medication,daily life ability and nursing satisfaction.
