摘要
目的观察CT引导下射频热凝术治疗带状疱疹后遗神经痛(PHN)的临床效果及安全性。方法以我院2017年1月至2018年12月收治的符合纳入选标准的52例PHN患者为研究对象,入组患者均进行CT引导下射频热凝术治疗,评估患者治疗疗效,采用视觉模拟疼痛(VAS)评分、麻木评分评估围术期入组患者疼痛程度、麻木症状,生命质量(QOL)评分评估患者生活质量,统计入组患者并发症发生情况。结果52例患者,治愈了12例,显效了20例,好转了16例,总有效率为92.31%,平均疼痛缓解时间为(3.15±1.48)d,平均结痂时间为(4.72±0.64)d;与术前相较,术后1周、1个月、6个月52例PHN患者VAS评分明显降低,QOL评分、麻木评分明显升高,差异有统计学意义(P<0.05)。52例患者总并发症发生率为26.92%(14/52)。结论CT引导下射频热凝术治疗PHN患者的临床疗效明确,在缓解患者疼痛及麻木症状、生活质量方面的积极作用明显,适于临床上推广应用。
Objective To observe the clinical curative effect and safety of CT-guided radiofrequency thermocoagulation(RFTC)for the treatment of post herpetic neuralgia(PHN).Methods A total of 52 PHN patients admitted to our hospital from January 2017 to December 2018 who met the inclusion criteria were enrolled.All patients underwent CT-guided RFTC.The curative effect was evaluated.Scores of visual analog pain scale(VAS)and numbness score were performed to assess pain level and numbness symptoms of patients during the perioperative period.The quality of life(QOL)scores were performed to evaluate the quality of life.The occurrence of complications was statistically analyzed.Results In the 52 patients,there were 12 cases cured,20 cases with marked effects,and 16 cases improved.The total response rate,average pain relief time,and average crusts time were 92.31%,(3.15±1.48)d and(4.72±0.64)d,respectively.Compared with those before surgery,the VAS scores of the 52 PHN patients were significantly decreased at 1 week,1 month and 6 months after surgery,while QOL scores and numbness scores were significantly increased(P<0.05).The overall incidence of complications in the 52 PHN patients was 26.92%(14/52).Conclusion The clinical curative effect of CT-guided RFTC is significant in PHN patients.Moreover,the positive effects are apparent in relieving pain,numbness symptoms and quality of life.
